To Contrast The Visual Quality Of ICL Implantation Combined With Modified LRI And TICL Implantation In The Treatment Of High Myopia With Astigmatism

Objective:To contrast the visual quality of ICL implantation combined with modified limbal relaxing incisions and TICL implantation in the treatment of high myopia with astigmatism.Method:From January 2022 to September 2022,we selected 36 patients(61 eyes)who were operated in our hospital.We divide those patients who with high myopia and astigmatism into two groups: lytic group(17 cases,31 eyes)and TICL group(19cases,30 eyes).The lytic group operated ICL V4 c implantation combined with modified LRI;the TICL group operated TICL implantation.Preoperative and postoperative 1 day,1 week,1 month,3 months,we test UDVA,SE,IOP and Average astigmatism of these patients.Higher-order aberration,modulation transfer funtion(MTF),strehl ratio(SR)were examined by OPD-ScanⅢ visual quality analyzer.At the same time,subjective visual quality of life questionnaire was conducted 3 months after surgery.Results:1 、 There were no significant differences in preoperative general information between the lytic group and the TICL group(All P > 0.05).2、Three months after surgery,the UDVA of two groups were(-0.01±0.06)log MAR、(-0.01±0.07)log MAR,the SE of the two groups were(-0.14±0.12)D and(-0.11±0.18)D,the average astigmatism of the two groups were(0.27±0.28)D and(0.26±0.25)D,and there were no significant difference between the two groups of those datas(all P > 0.05).There also no clear difference in average value of intraocular pressure between the two groups at each time point before and after surgery(all P > 0.05).3、The total aberration root mean square results : Three months after surgery,the total high-order aberration 、 trefoil aberration in the lytic group were increased compared with those before surgery,with statistical significance(all P < 0.05);the coma of TICL group was(0.052±0.032)before surgery and(0.085±0.036)at 3months after surgery,the coma of TICL group increased significantly after surgery(P< 0.05).Three months after surgery,lytic group and TICL group were compared between groups:the total high-order aberrations of the two groups were(0.361±0.193)and(0.248±0.066),and the difference was statistically significant(P=0.025).The trefoil aberrations of the two groups were(0.262±0.160)and(0.146±0.067),with statistical significance(P=0.006).Coma aberrations of the two groups were(0.064±0.040)and(0.085±0.036),and the difference was statistically significant(P=0.003).There were no significant differences in the spherical aberration、MTF and SR between the two groups(all P > 0.05).4、Three months after surgery,both patients in the two groups had good distance vision.There were no significant differences in glare and halo symptoms between groups(all P>0.05).The symptoms of dry eye and visual fatigue were mild in both groups,which did not affect daily vision.There were no symptoms such as diplopia and abnormal color vision in the two groups.Conclusion:ICL implantation combined with modified LRI and TICL implantation can obtain good visual quality.
