Objective:To determine the affective nursing intervention program of traditional Chinese medicine for patients with severe hepatitis,and study the influence of affective nursing of traditional Chinese medicine on anxiety and depression of patients with severe hepatitis,so as to provide reference for the future clinical application of the program,but also to alleviate the negative emotions of patients with severe hepatitis,improve their liver function,and improve patient satisfaction to provide evidence-based evidence.Methods:1.Literature analysis: the index system of TCM emotional nursing intervention program for patients with severe hepatitis was preliminarily formulated by consulting domestic and foreign literatures related to TCM emotional nursing,the relationship between psychology,emotion and liver of patients with severe hepatitis.2.Research group discussion method: the index system of TCM emotional nursing intervention program for patients with severe hepatitis was initially formulated,and the first round of expert consultation table was formed through the research group discussion.3.Delphi method: the number of experts and inclusion criteria were determined,and the experts who met the requirements were selected as the consulting experts for the construction of the intervention program.After two rounds of expert consultation,the TCM emotional nursing intervention program for patients with severe hepatitis was finally determined.4.Randomized control method: 64 patients with severe hepatitis were randomly divided into two groups: control group(32 cases)and observation group(32 cases).Patients in the control group received routine nursing,while patients in the observation group received TCM emotional nursing intervention program including doubt explanation method,emotional counterbalance method,empathy therapy and implication therapy on the basis of routine nursing.Self-rating anxiety scale(SAS),self-rating depression scale(SDS)and hospital self-made patient satisfaction questionnaire for nurses were used as evaluation forms,and the questionnaires were sent to the two groups of patients in the form of questionnaire star to fill in online,and the level of liver function was monitored at the same time.The anxiety score,depression score,total bilirubin(TBIL)and alanine aminotransferase(ALT)of all patients on the first day of admission and the 14 th day of admission were collected,and the patients’ satisfaction with nurses on the 14 th day of admission was evaluated.The nursing effects of the observation group and the control group were analyzed and compared.Results:1.Literature analysis: a total of 15 directly related literatures were finally included.Based on literature review and group discussion,the index system of TCM emotional nursing intervention for patients with severe hepatitis was preliminarily constructed,including 4 first-level indicators,9 second-level indicators and 25third-level indicators.2.Delphi expert consultation: the TCM emotional nursing intervention program for patients with severe hepatitis was constructed.The final indicators included 4first-level indicators,8 second-level indicators and 17 third-level indicators.The enthusiasm of experts in the two rounds of consultation was 100% and 90%,respectively.The authority coefficients were 0.87 and 0.90;After two rounds of expert consultation,the Kendall harmony coefficient of the importance of indicators at all levels ranged from 0.177 to 0.835.The chi-square test showed that the difference was statistically significant(p<0.05),indicating a high degree of coordination of expert opinions.The coefficient of variation of the importance and feasibility of each item ranged from 0.00 to 0.20,and the expert opinions were relatively concentrated.3.Baseline comparison between the two groups on the first day of admission,There were no significant differences in gender,age,education level,self-rating Anxiety scale(SAS)score(p=0.906),self-rating Depression Scale(SDS)score(p=0.600),total bilirubin(p=0.710)and alanine aminotransferase(p=0.619)between the observation group and the control group(p>0.05).4.Comparison between the two groups of patients on the 14 th day: The scores of self-rating Anxiety Scale(SAS)(p<0.001),Self-rating Depression Scale(SDS)(p<0.001),total bilirubin(TBIL)(p=0.020)and alanine aminotransferase(ALT)(p=0.019)in the observation group were lower than those in the control group.The score of patient satisfaction was higher than that of the control group(p<0.001),and the difference was statistically significant.5.Comparison of patients before and after nursing in the observation group: The scores of self-rating Anxiety Scale(SAS)(p<0.001),Self-rating Depression Scale(SDS)(p<0.001),total bilirubin(TBIL)(p<0.001)and alanine aminotransferase(ALT)(p<0.001)were lower on the 14 th day than on the 1st day,and the difference was statistically significant.6.Comparison of the difference of anxiety and depression scores between the two groups before and after intervention: the difference in the observation group was greater than that in the control group(p<0.001),and the difference was statistically significant.Conclusion:1.The emotional nursing intervention scheme of traditional Chinese medicine is scientific and can be used as the basis for psychological nursing of patients with severe hepatitis..2.There was no difference in the baseline level of patients with severe hepatitis at admission.Patients in both groups had moderate anxiety and mild depression,and adverse mood needed to be improved.3.Affective nursing intervention program of traditional Chinese medicine can significantly reduce anxiety and depression in patients with severe hepatitis,improve liver function and improve patients’ satisfaction with nursing staff.4.On the basis of conventional nursing,the clinical effect of emotional nursing intervention scheme of traditional Chinese medicine is better than that of conventional nursing alone.
