| Objective:Exploring the relationship between T cell subpopulations and immunoglobulins in different stages and groups of COPD patients and the severity of the disease.Methods:1.Collect 40 patients with stable COPD,45 patients with acute exacerbation,and 22 corresponding healthy subjects(control group)who visited the Third People’s Hospital of Jiujiang.COPD patients were divided into AB group(Agroup+B group)and E group according to GOLD2023.2.Pulmonary function tests were performed at presentation in the COPD stabilization group and control group,pulmonary function tests were performed at discharge in the COPD exacerbation group,and peripheral blood was collected from all patients for lymphocyte and immunoglobulin testing.The pulmonary function test was detected by the German Kangxun pulmonary function instrument,CD3+ T cells,CD4+ T cells and CD8+ T cells were detected by the American CYTOMICS FC5003.3 flow cytometer,and serum immunoglobulin was detected by Olympus biochemical instrument by immunoscattering turbidimetry.3.T test,Chi-square test and correlation analysis were used to study the differences between the above indexes and COPD staging and GOLD group.Results:1.The level of CD3+ T cells in the COPD stable group was 673.27±251.28cells/μL,the level of CD4+ T cells was 412.47±163.13 cells/μL,the level of CD8+ T cells was 240.33±114.98 cells/μL,and the level of CD3+ T cells in the COPD exacerbation group was 500.15±269.62 cells/μL,and the level of CD4+ T cells was289.15±171.75 cells/μL and CD8+ The level of T cells was 185.05±118.33 cells/μL,compared with the stable phase and the acute exacerbation phase,the level of CD4+ T cells in the stable group was significantly higher than that in the acute exacerbation stage(P<0.05),there was no significant difference in the level of CD3+ T cells and CD8+ T cells between the two groups(P>0.05),and the level of CD3+ T cells in the control group was 1382.89±236.46 cells/μL,and the level of CD4+ T cells was741.33±158.37 cells/μL and CD8+ The level of T cells was 545.39±90.14 cells/μL,which was significantly higher in the control group than in the COPD stable and exacerbation groups(P<0.05).There was no significant difference in the percentage of T cells in the blood of COPD patients in the acute exacerbation and stable COPD groups,and the percentage of CD3+ T lymphocytes and CD4+ T lymphocytes in the control group was significantly higher than that in the COPD stable group and acute plus recombination.There was no significant difference in the CD4/CD8 ratio between the three groups.2.The levels of IgG and IgA in peripheral blood of control groups were10.13±3.17g/L and 1.63±0.87g/L,respectively.The levels of IgG and IgA in peripheral blood of patients with acute exacerbation were 14.70±4.73g/L and2.66±1.29g/L,The exacerbation group was significantly higher than the control group(P<0.05).3.The mMRC scores of AB group were significantly lower than those in group E,and the levels of FEV1/FVC,FVC%,FEV1%,CD3+ T cells,CD4+ T cells and CD8+ T cells were significantly higher than those in group E(P<0.05).4.The results of Pearson correlation analysis showed that the fitting coefficients R2 of FVC% with CD3+,CD4+ and CD8+ T cell levels were 0.6849(P<0.0001),0.6198(P<0.0001)and 0.6329(P<0.0001),respectively,and the fitting coefficients R2 of FEV1% and CD3+,CD4+ and CD8+ T cell levels were 0.7840(P<0.0001),0.7336(P<0.0001)and 0.7015(P),respectively <0.0001),FVC% and FEV1% were positively correlated with the number levels of CD3+,CD4+,and CD8+ T cells in the blood.Conclusion:The number of T cells subgroups in peripheral blood is an indicator to measure the severity of COPD patients.The more severe the symptoms of COPD patients,the less the number of peripheral blood CD3+,CD4+ and CD8+T lymphocytes,and it is highly correlated with the FEV1% and FVC% of COPD patients. |
