In recent years,As the comprehensive reform of public hospitals continues to deepen,various regions across the country are gradually implementing the classification reform of public hospitals,strengthening hospital management,and enhancing the diagnostic and treatment capabilities of doctors.The clinical autonomy and corporate autonomy of clinicians have not been truly implemented,and the professional autonomy of clinicians is still largely affected by state intervention and government control.In the actual work environment,clinicians’ professional autonomy still faces great challenges.At the same time,under the increasing demand for medical and health services,the clinician community has been in a high-load,high-risk,and high-intensity work state for a long time,resulting in a crisis in the professional commitment of the clinician community in recent years.Based on a literature review,this study took clinicians from six public hospitals in Shanghai as the research objects,used questionnaire survey method to study the impact of professional autonomy on clinicians’ professional commitment,took job prosperity,occupational risk perception,and defensive medical behavior as the mediating variables,and introduced two moderating variables,clinicians’ sense of organizational support and social identity,in an attempt to establish a theoretical model between occupational autonomy and clinicians’ professional commitment.Through the questionnaire survey of the research variables,through the analysis of the questionnaire data of the research subjects,the model hypothesis of the two paths is further proposed,and the hypotheses are verified one by one by using questionnaire star,SPSS 25.0,process and other software,and the relationship between the two hypothesized paths is tested empirically.Through empirical research,it is found that occupational autonomy has a positive and significant impact on clinicians’ professional commitment,occupational autonomy has a positive and significant impact on work prosperity and organizational support,work prosperity and social identity have a positive and significant impact on clinicians’ professional commitment,and occupational autonomy has a negative and significant impact on occupational risk perception and defensive medical behavior.At the same time,through the test of mediation effect,it is found that work prosperity plays a partial mediating role between occupational autonomy and professional commitment,and occupational risk perception and defensive medical behavior play a chain mediating role between occupational autonomy and clinicians’ professional commitment.In addition,social identity plays a significant positive role in moderating between job prosperity and career commitment.Finally,based on relevant literature,empirical research results and the author’s work experience,this paper puts forward relevant suggestions to enhance clinicians’ professional autonomy and professional commitment.The results of this study enrich the research on professional autonomy in the clinician community and play a certain degree of reference in enhancing the professional commitment of clinicians.
