| Objective: This study aims to explore the similarities and differences of influencing factors of medical-seeking behaviors,and anxiety and depression disorders between cardiovascular outpatients in tertiary hospitals and primary hospitals.Besides,the paper also analyzes the effects of anxiety,depression,and somatization disorders on medical-seeking behavior.Methods: Questionnaires were issued to outpatients in the cardiology department from April 2019 to March 2020 in a primary hospital(Liling Hospital of Traditional Chinese Medicine),and a total of1767 copies were recovered.According to the different registration types,the patients were divided into 3 groups.The demographic characteristics,economic and cultural levels,family relationships and other general information of the patients were collected.Furthermore,the scores to access the anxiety and depression level including Generalized Anxiety Disorder(GAD-7)and Patient Health Questionnaire-9items,PHQ-9),and Patient Health Questionnaire-15(PHQ-15)to access somatic symptoms are also collected.The above results were compared with the patients in a tertiary hospital,Xiangya Secong Hospital of Central South University.We analyzed the mood disorders(anxiety,depression)of patients in different types of outpatient clinics and compared the differences in patients’ mood disorders between tertiary hospitals and primary hospitals.Results:(1)According to the general information of the patients in tertiary hospitals and primary hospitals,there are statistically significant differences in occupation,education level,income,residence,and medical insurance between the two groups of patients,but no significant differences in age,gender,marital status,and family members.(2)For the cardiovascular patients of the tertiary hospital,the overall anxiety incidence of is 41.13%,the incidence of depression is 34.66%,and the positive rate of somatization symptoms is 66.11%;while for the cardiovascular patients of the primary hospital,the overall incidence of anxiety is 44.50%,and the overall incidence of depression is 37.40%,and the positive rate of somatization symptoms is 68.20%.There is no significant difference in anxiety,depression,and somatization symptom scores between the two groups.(3)The levels of anxiety and depression of the patients increases among outpatient types of attending physicians,chief physicians,expert outpatient clinics,and special needs outpatient clinics.There are significant differences among groups(P<0.05).The anxiety scores of the attending physician outpatient,chief physician outpatient and expert outpatient clinics of the tertiary hospital are lower than the corresponding outpatient types of the primary hospital,and the differences are statistically significant.The depression scores of patients in the tertiary hospital are lower than those in the primary hospital,but the difference is not statistically significant;the somatic symptom scores of patients in the attending physician outpatient clinic,chief physician outpatient clinic and expert outpatient clinic in the tertiary hospital are lower than the corresponding outpatient clinic types in the primary hospital,and the differences are statistically significant.(4)For patients in the tertiary hospital,the anxiety score is influenced by gender,occupation,marital status,number of family members,and area of residence;the depression score is influenced by occupation and marital status;the somatic symptoms score is influenced by occupation,education degree,income,composition of family members and area of residence.For patients in the primary hospital,the anxiety score is influenced by marital status and health insurance;the depression score is influenced by marital status,health insurance.The somatic symptoms score is influenced by education level,marital status,medical insurance,and area of residence.And the medical behavior of patients is mainly related to their gender,marital status,area of residence and whether they have medical insurance.Conclusion: In both tertiary hospitals and primary hospitals,cardiology patients have a high prevalence of anxiety and depression.There are some common influencing factors of medical-seeking behaviors,and anxiety and depression.There are certain differences in patients’ anxiety and depression in different types of outpatient clinics,suggesting that the importance of developing psycho-cardiology clinics in hospitals. |
