| Objective:To explore the effect of intraoperative biliary internal stent placement on biliary tract complications after orthotopic liver transplantation for adult,and to study the application value of biliary internal stent in liver transplantation,so as to provide reference for the diverse selection of biliary tract reconstruction in clinical work,and examined risk factors of biliary tract complications after liver transplantation.Methods:A retrospective cohort study was conducted on the biliary reconstruction methods and postoperative biliary complications of251 patients who underwent a orthotopic liver transplantation in The Second Xiang Ya Hospital of Central South University from January 2020 to September 2021.According to inclusion criteria and exclusion criteria,194 patients were screened out and divided into 2 groups based on whether a biliary internal stents was placed intraoperative.72 patients with biliary internal stent were enrolled in the observation group,and 122 patients without biliary internal stent were enrolled in the conventional group.The incident of the biliary tract complications between the two groups were compared.Patients were divided into 2 groups based on whether biliary complications occurred,independent risk factors was analyzed by univariate and multivariate analysis.Results:Among the 194 patients in this study,13 patients(6.7%)developed biliary tract complications,including biliary stenosis in 8 cases and biliary leakage in 5 cases.In the observation group,there were 2 patients with biliary leakage,and there were 11 patients in the conventional group with biliary complications,including biliary stenosis in 8 cases and biliary leakage in 3 cases.No significant difference was found in the overall biliary complications(p=0.137),and biliary leakage(p=1.0)between the two groups,while the incidence of biliary stenosis was statistically significant(p=0.027).In multivariate logistic regression analysis,the use of biliary stents was the only protective factor for overall biliary complications.Conclusions:The use of biliary internal stent during liver transplantation can effectively reduce the incidence of biliary stenosis,and Intrabiliary stents also act as a protective factor to reduce the overall risk of biliary complications,further research is needed to draw definitive conclusions.There are 5 figures,10 tables,50 references in this paper. |
