Objective:Thyroid cancer is the most common malignant tumor in endocrine tumors,The preferred treatment of thyroid cancer is early surgical treatment,and thyroid cancer is prone to cervical lymph node metastasis.In recent years,the scope of cervical lymph node dissection for thyroid cancer and whether preventive dissection are controversial,lymph node assessment methods include b-ultrasound,ct,magnetic resonance,needle biopsy,etc.At present,the expression of most small RNAs is regulated in thyroid cancer,and mirna plays a key role in the development and development of thyroid cancer,and molecular markers have become a hot topic in thyroid cancer.Therefore,the relationship between the expression levels of miR-221 and miR-222 in thyroid serum and cervical lymph node metastasis in patients with micropapillary carcinoma was investigated,prepredict the presence of cervical lymph node metastasis in micropapillary thyroid carcinoma and the operation plan was reasonably formulated.To provide new methods for the development and diagnosis of thyroid micropapillary carcinoma.Methods:(1)From April 2022 to November 2022,the serum specimen of patients who underwent surgery in Liuzhou People’s Hospital and were pathologically diagnosed as benign thyroid tumor and micropapillary thyroid cancer were selected as the research objects.A total of 90 patients were included in this experiment,of which 30 patients were control group(patients with benign thyroid tumors);30 patients were experimental group 1(papillary thyroid microcarcinoma without cervical lymph node metastasis),and 30 patients were experimental group 2(papillary thyroid microcarcinoma with cervical lymph node metastasis).(2)The cycle number ct value of miR-221 and miR-222 were calculated by Reverse Transcription-Polymerase Chain Reaction method,and 2^-ΔΔCt was used to calculate the expression amount again.Clinical data were collected and sorted out: age,sex,diameter of cancer foci,number of cancer foci,membrane invasion,cancer focus location,cervical lymph nodes and number of lymph node metastases.(3)Analysis of the expression levels of miR-221 and miR-222 between groups.The expression level of the target gene was analyzed with each clinical data.Results:(1)The expression level of serum mir-221 was statistically significant at (?)±s0.69± 0.60,1.25 ± 0.93 and 1.81 ± 1.30(F=9.591,P=0.000 <0.05)(2)The expression level of serum mir-221 was statistically significant at x±s 0.35 ± 0.19,0.74 ± 0.81,1.28 ± 0.97(F=11.726,P=0.000 <0.05)(3)The expression levels of serum miR-221 and miR-222 in patients with papillary thyroid microcarcinoma of the thyroid gland were significantly higher than those in patients with benign thyroid tumors(p <0.05).(4)The significant difference in serum miR-221 in thyroid patients without cervical lymph node metastasis and those with cervical lymph node metastasis(SE=0.25,P=0.03<0.05).(6)The significant difference in serum miR-222 in thyroid patients without cervical lymph node metastasis and those with cervical lymph node metastasis(SE=0.19,P=0.007<0.05).(5)The difference in expression levels of serum mir-221 in patients without cervical lymph node metastasis(SE = 0.25,P=0.03 <0.05)and mir-222(SE = 0.19,P =0.007 <0.05).(6)The expression level of serum miR-221 and miR-222 was statistically significant compared with the number of cancer foci,diameter of cancer foci,capsule invasion,and cervical lymph node metastases(P <0.05).(7)The expression levels of miR-221 and miR-222 were not statistically significant with sex,age and carcinoma(P > 0.05).(8)Serum miR-221 had diagnostic value for cervical lymph node metastasis in papillary thyroid microcarcinoma(AUC=0.799,P <0.05).(9)Serum miR-222 had diagnostic value for cervical lymph node metastasis in papillary thyroid microcarcinoma(AUC=0.828,P <0.05).(10)Combined serum miR-221 and miR-222 have diagnostic value for cervical lymph node metastasis in thyroid micropapillary carcinoma(AUC=0.894,P <0.05).Conclusion:(1)The expression levels of serum miR-221 and miR-222 could have a high diagnostic value for the evaluation of cervical lymph node metastasis in papillary thyroid microcarcinoma.(2)Serum miR-221,miR-222 may be used as biomolecular markers for assessing cervical lymph node metastasis in papillary thyroid microcarcinoma.(3)There was a correlation between the expression level of serum miR-221 and miR-222 and the number of cancer foci,diameter of cancer foci,capsule invasion,and number of cervical lymph node metastases.(4)There was no correlation between the expression levels of serum miR-221 and miR-222 and gender,age,or cancer focal sites.
