Clinical Study On The Treatment Of Lower Limb Spasticity After Spinal Cord Injury By Deeply Needing On Baliao Points

Purpose By comparing the clinical efficacy of deep needing on baliao points and baclofen on patients with lower limb spasm after Spinal Cord Injury(SCI),it is expected to broaden the methods and ideas of acupuncture for treating lower limb spasm after spinal cord injury.MethodThe cases of lower limb spasm after SCI seen in the Second Affiliated Hospital of Anhui University of Traditional Chinese Medicine from April 2022 to January 2023 were included.The included cases were divided into treatment group(deeply needing on baliao points group)and control group(baclofen group),with a total of 62 cases.Both groups were given routine acupuncture and rehabilitation treatment for the lower extremities.The control group received daily oral baclofen on top of basic treatment.On basic treatment,the treatment group was treated with deep needing on eight points.Both groups were acupuncture from Monday to Saturday for a total of 4weeks.Tension grading(MAS),the degree of spasm(CSS),and range of motion(ROM)were evaluated in two groups of patients before and after intervention,and 1 month after the course.The F-wave time dispersion and the average occurrence rate were recorded and calculated.The data were collated and analyzed by SPSS26.0,comprehensively comparing the differences between the two groups,and calculating the clinical response rate.Result1.After treatment and at 1-month follow-up,TCM syndrome scores,MAS scores and CSS scores in the treatment and control groups were significantly lower,and hip flexion,knee flexion and ankle dorsiflexion ROM was significantly higher than before treatment(P<0.01).Compared with the control group,the treatment group significantly improved TCM syndrome scores(P<0.01)and had advantages in improving MAS score,CSS score,hip flexion,knee flexion and ankle dorsiflexion ROM(P<0.05).2.Clinical effective rate: After treatment,the effective rate of the treatment and control groups was 83.87% and 58.06%,respectively,while at the follow-up time,the effective rate of the two groups was 87.10% and 64.52% respectively,so the effective rate of the treatment group was higher and it was statistically significant(P<0.05).3.F wave of the surface electromyography: The time dispersion and mean occurrence of F wave in the two groups after treatment and follow up were significantly lower(P<0.01);The comparison between the groups showed that the time dispersion and average occurrence of F wave in the treatment group were lower than that of the control group,and the average occurrence of F wave in the treatment group was significantly lower than that of the control group after treatment,with statistical difference(P<0.05).4.There were only 3 cases of local blood stasis after acupunture in the treatment group,which dissipated spontaneously without intervention,and both treatment measures were safe and reliable.Conclusion1.Deeply needing on baliao points has more advantages in improving TCM syndromes,reducing MAS score and CSS score,improving ROM score;2.Deeply needing on baliao points can reduce F-wave time dispersion and average occurrence rate,and has more reliable efficacy in patients with lower limb spasm after SCI.
