| Objective:This article mainly studies the therapeutic effect of Xiao Chai Hu Tang plus minus in patients with vertebrobasilar insufficiency(VBI)vertigo.The different results before and after treatment of the two groups were observed experimentally,and the differences in treatment response rate,TCM symptoms and signs,dizziness handicap inventory(DHI)score,TCD blood flow parameters,Nitric oxide(NO),and Endothelin-1(ET-1)indexes were analyzed.Methods:Through the comparison of results,we can understand the therapeutic effect of Xiaochai Hu Tang on VBI,better play the role of Xiao Chai Hu Tang in the clinic,and explore more comprehensive treatment options.1.Grouping: 63 patients with VBI were selected by random number table method,and randomly divided into two groups,one group was 32 cases in the control group and the other group was 31 cases in the observation group.The observation group was treated with internal administration of Xiaochai Hutang plus minus flunarizine hydrochloride capsules,while the control group was only treated with flunarizine hydrochloride capsules.2.Treatment indicators: patients in both groups were treated with four courses of continuous treatment,each course of 8 days.After treatment,the effective rate,TCM syndrome and sign scores,DHI scores,blood flow rate,NO and ET-1 were compared between the two groups before and after treatment.Results:1.Feasibility comparisonThere was no significant difference in gender,age,course of disease and other observation items between the two groups(P>0.05),which were comparable.2.Efficacy evaluation:(1)Effective rate: the total effective rate of the treatment group was 96.67%,which was higher than that of the control group(83.33%),the difference was statistically significant(P<0.05).(2)TCM syndrome and sign scores: after treatment,the scores of dizziness,head swelling or pain,less sleep and more dreams,bitter mouth and dry mouth,impatience and irritability,and attack frequency were significantly improved in the two groups(P<0.05);The comparison between the two groups showed that the clinical syndrome observation group had obvious clinical effects,and the differences were statistically significant(P<0.05);The improvement of dizziness and dizziness syndrome score in the observation group was significantly better than that in the control group,and the difference was statistically significant(P<0.01);It showed that the observation group was better than the control group in improving the main symptoms and some accompanying symptoms of VBI.(3)DHI scores: after treatment,the scores of physical symptoms,functional symptoms and emotional symptoms of the two groups were significantly improved compared with those before treatment,with statistically significant differences(P < 0.05);After treatment,there was no significant difference in emotional symptom score between the two groups(P>0.05).The scores of physical symptoms and functional symptoms in the observation group were significantly better than those in the control group(P<0.05).(4)Blood flow rate:the blood flow velocity of vertebral artery and basilar artery was significantly improved after treatment in both groups(P<0.05),and the difference was statistically significant.Compared between the two groups,the blood flow velocity of vertebral artery and basilar artery in the observation group after treatment was higher than that in the control group,and the difference was statistically significant(P< 0.05).(5)NO: After treatment,the level of NO in the two groups was significantly higher than that before treatment,and the difference was statistically significant(P< 0.05);After treatment,the level of NO in the observation group was significantly higher than that in the control group,and NO was also higher than that in the control group,the differences were statistically significant(P<0.05).(6)ET-1: After treatment,compared with before treatment,the level of ET-1 in the two groups decreased,and the difference was statistically significant(P < 0.05);After treatment,the ET-1 of the observation group was significantly lower than that of the control group,and the ET-1 was also higher than that of the control group,the difference was statistically significant(P<0.05).(7)Safety: during the treatment,no discomfort and side effects were found in the two groups.There were no significant changes in laboratory tests,electrocardiogram and other safety indicators before and after treatment.Conclusion:The modified Xiaochaihu decoction can effectively improve the main symptoms of vertebrobasilar insufficiency vertigo caused by liver stagnation and heat,relieve some accompanying symptoms,and can be safely and effectively used in clinical practice. |
