| Acne vulgaris is the most common chronic inflammatory disease in dermatology,and the imbalance of sebum homeostasis is a key factor in acne.Through clinical practice and previous studies,it has been observed that acne with the same severity may have the following two different pathogenic patterns: one is a long-term stable periodic attack,that is,there is no significant difference in the severity grade at the peak of acne onset for a long time;The other pathogenic pattern,belongs to the breakthrough cycle aggravated attack,that is,at a certain point in time,acne suddenly aggravated,and the severity of subsequent attacks is significantly higher than the previous base level.There are few studies on whether the two types of pathogenesis are related to lipid.Therefore,this study compared acquisition amount of comedo from different facial zones(forehead,nose and its sides,the place around the mouth and cheeks)for acne patients with pathogenesis modes,and combined spectrometry with liquid chromatography-mass spectrometry(LCMS)technique was used to conduct the lipids of the comedoes in different facial zones of acne patients with different pathogenesis modes,focusing on the differences in the lipidomics of cheek comedo,in order to further explore the pathogenesis of acne and inflammation,and provide reference for clinical treatment strategies.Research methods:1.By comparing the amount of comedo acquisition in different facial zones of acne patients with different pathogenic patterns,the differences in the amount of comedo acquisition in different facial zones of acne patients with different pathogenesis modes are discussed,providing evidences for the treatment of acne vulgaris patients with different distribution modes and their facial zones.2.In this study,LC-MS lipomic analysis technology was used to identify and quantitatively analyze the lipid content of cheek comedo in acne patients with different pathogenesis modes,screen out different lipid metabolites of acne vulgaris with different pathogenesis modes,and explore the role of lipids in acne vulgaris with different pathogenesis modes,especially in the process of acne trigger and exacerbation.Research results and conclusions:1.Difference analysis of comedo acquisition between breakthrough group and Periodic group(1)There was no significant correlation between gender and acne acquisition in breakthrough group and Periodic group.(2)The age of the breakthrough group was negatively correlated with the amount of comedo acquired in the forehead,nose both its sides and cheeks.In the periodic group,there was no significant correlation between age and comedo acquisition in each facial zones.(3)The per capita comedo acquisition amount in all facial zones of acne patients in the breakthrough group was greater than that in the acne patients in the periodic group.The total comedo acquisition amount in different facial zones of patients in the two groups was compared between the two groups.The total comedo acquisition amount in cheeks and around the mouth of the two groups was statistically different between the two groups,and the total comedo acquisition amount in cheeks of the two groups of grade II acne patients was statistically different between the two groups.2.Lipidomics analysis of cheek acne in breakthrough group and periodic group(1)There was no significant difference in lipid content in unit comedo between periodic group and breakthrough group.(2)There were significant differences in lipid subtypes and lipid molecules in unit comedone between the periodic group and breakthrough group.Compared with the periodic group,the contents of 6 lipid subclasses such as Phosphatidylserine(PS),phosphatidylglycerol(PG)and Cardiolipin(CL)were significantly increased in the breakthrough group,Among them,lipid subclasses whose main function is proinflammatory include PS,PG,CL,Sulfatide(,ST),Monosialo trihexosyl ceramide(GM3);In addition,the contents of 10 lipid subclasses such as Phosphatidylinosital biphosphate(PIP2),Phosphatidic acid(PA)and Hexosyl ceramide(Hex1SPH)were significantly reduced.Besides,137 lipid molecules were significantly up-regulated and 193 were significantly down-regulated.Research conclusion:(1)Comedo around the mouth showed different characteristics in the development of Comedo,that is,comedo around the mouth did not decrease with the increase of age.(2)In the process of acne activity and aggravation,there are obvious lipid composition changes in cheek comedo.Compared with the patients with periodic stable disease,the lipid metabolites PS,PG and CL in the acne of severe patients promote inflammation and aggravate oxidative stress,which may play an important role in the process of triggering and aggravating acne vulva.Besides,there are other factors involved in acne triggering and aggravating. |
