Objective : To explore the effect of application of health education videos combined with clinical pathways in perioperative renal transplantation patients based on“knowledge,belief and practice” model.Methods: Random-controlled trial with cluster sampling.Health education videos for perioperative kidney transplantation patients were constructed based on the "knowledge,belief and practice" model.Ninety-eight kidney transplantation patients eligible for inclusion were recruited from the organ transplant center of a tertiary hospital in Sichuan,China.The patients were randomly divided into 14 clusters with 7patients in each cluster,including 7 intervention clusters and 7 control clusters.After the videos were sent to the patients in the intervention group,the in-charge nurse checked the patients’ self-learning status according to the path table,and provided targeted explanations and supplements based on patients’ answers,while the control group received routine health education.At the time of inclusion in the study and on the8 th day after surgery,VAS-A was used to assess anxiety of patients.On the 8th day after surgery,a self-developed "disease related knowledge" questionnaire was used to evaluate patients’ knowledge on disease.On the day of discharge,the "health education satisfaction" questionnaire and the "Care Transitions Measure-15" were used to assess patients’ satisfaction and transitional care quality.A self-constructed “ patient data collection form” was used to collect general data,information during hospitalization and post-discharge.At 1 month and 3 months after discharge,data on unplanned clinic visits,readmission and compliance with immunosuppressants(using BAASIS)were collected via phone.Student t test was used to analyze continuous data with normal distribution.Mann-Whitney U test was used for the analysis of non-normally distributed continuous data,and Pearson Chi-square test or Fisher exact test for the analysis of categorical data.Results: The intervention group had statistically better results in disease-related knowledge,quality of transitional care,BAASIS score at 1 month after discharge,degree of anxiety,patient satisfaction,time to first get out of bed after surgery,time to first eat after surgery,time to first pass gas after surgery,readmission at 3 months after discharge and unplanned clinic visits at 3 months after discharge than the control group(p<0.05).The intervention group had lower average hospitalization cost,shorter average length of hospital stay and average length of hospital stay after surgery than the control group,but the difference was not statistically significant.There were no statistically significant differences in readmission and unplanned clinic visits at 1 month after discharge,BAASIS score at 3 months after discharge,postoperative infection,postoperative bleeding and postoperative deep vein thrombosis between groups.Conclusion: The application of knowledge,belief,and practice based education videos combined with clinical pathways in kidney transplantation patients can achieve short-term and long-term effects.The intervention of the study does not increase the workload of nurses,and have good clinical applicability.
