Objective:To investigate the effect of Castor single-branched stent in the treatment of aortic dissection.Methods:The clinical data of 42 patients with aortic arch disease and insufficient landing zone treated in the First Affiliated Hospital of Anhui Medical University from September 2020 to September 2022 were retrospectively analyzed.All patients underwent Castor single-branch stent implantation.According to the results of preoperative aortic CTA,the appropriate proximal diameter,distal diameter,main body,length,distal diameter and length of branch stent and backward displacement of branch were selected.Statistical analysis was performed using SPSS 26.0,continuous variables were expressed as mean ± SD,and categorical data were expressed as counts.Changes in the diameter of the true and false lumen before and after surgery were assessed using a paired t-test.A p value of less than 0.05 was considered statistically significant.Results:There were 37 males and 5 females with a mean age of 55 years.There were40 cases of acute aortic dissection and 2 cases of aortic arch aneurysm.The mean time was 129±52 minutes.The aortic diameter at left subclavian artery before and after operation was(27.7±3.7mm,30.4±3.2mm;p<0.01).The diameter of LSA before and after operation was(9.6±1.5mm,11.6±1.4mm;p<0.01).In 40 patients with acute aortic dissection,the diameter of the true lumen of the isthmus aorta was compared before and after operation(24.7±10.2mm,33.0±7.5mm;p<0.01).The diameter of the false lumen of the isthmus aorta was(22.9±14.3mm,12.4±12.1mm;p<0.01).The diameter of the aortic true lumen at the bifurcation of pulmonary artery before and after operation was(21.7±8.2mm,28.7±4.3mm;p<0.01).The diameter of aortic false lumen at the bifurcation of pulmonary artery before and after operation was(20.1±11.7mm,11.4±9.5mm;p<0.01).The diameter of aortic true lumen at transverse septum before and after operation was(18.9±9.4mm,25.3±6.7mm;p<0.01).There was no significant difference in the diameter of the false lumen between preoperative and postoperative(19.7±11.8mm,13.8±11.0mm;p<0.01).The comparison results showed that the LSA and the aortic vascular lumen at LSA were enlarged after operation.In 40 patients with acute aortic dissection,the true lumen was enlarged and the false lumen was reduced at the aortic isthmus,pulmonary artery bifurcation and transverse septum after operation.Conclusions:1.Castor single-branched stent has a higher prognosis rate for aortic dissection patients with insufficient healthy landing zone distance.2.Castor single-branched stent has a low incidence of complications such as endoleak,neurological symptoms and re-dissection after aortic dissection.
