Objective: To further investigate the possible correlation between coronary atherosclerotic heart disease and periodontal conditions by investigating the differences between salivary cortisol levels in patients with and without coronary atherosclerotic heart disease,as well as saliva as a new sample for screening coronary atherosclerotic heart disease The possibility of salivary cortisol levels as a new indicator for screening for coronary atherosclerotic heart disease.Methods: A total of 89 consecutive patients who visited the Department of cardiovascular medicine at the affiliated Chaohu Hospital of Anhui Medical University between December 2021 and may 2022 were selected and divided into coronary atherosclerotic heart disease(CHD)group as well as non coronary atherosclerotic heart disease group according to their coronary angiographic findings,and their salivary cortisol levels were measured using enzyme-linked immunosorbent assay(ELISA),respectively,and the periodontal conditions of the subjects were examined by the periodontal bleeding index(BI)Periodontal health was evaluated by periodontal probing depth(PD),attachment loss(AL),calculus index(CI)and oral health impact profile scale(OHIP-14).Differences in the general condition of the study subjects,salivary cortisol levels,and periodontal conditions between the two groups were analyzed,and the relationships between the above indicators and the occurrence and development of coronary atherosclerotic heart disease and the correlation between coronary atherosclerotic heart disease and periodontal disease were analyzed.Results: There were no significant differences in the general conditions(age,sex composition ratio,smoking rate,alcohol consumption rate,and history of hypertension)between the coronary atherosclerotic heart disease group and the non coronary atherosclerotic heart disease group,and the periodontal conditions: periodontal bleeding index(BI),periodontal probing depth(PD),attachment loss(AL),and dental calculus index(CI)(P < 0.05),The salivary cortisol level in the CHD group was significantly higher than that in the non CHD group(P < 0.01);Salivary cortisol levels,loss of periodontal attachment and oral health affect status were significant risk factors for CHD(P < 0.05);Salivary cortisol levels had high diagnostic efficacy for coronary atherosclerotic heart disease with an optimal cut-off value of 7.38 nmol / L,95% CI(0.830-0.956),and its AUC was 89.3%.Conclusions:(1)Elevated salivary cortisol levels have implications for the development of coronary atherosclerotic heart disease,poor periodontal status;(2)Salivary cortisol levels,loss of periodontal attachment,and magnitude of oral health behavior effects may further increase the risk of CHD;(3)Salivary cortisol levels have the potential to serve as a test marker for risk factors in the evaluation of coronary heart disease.
