| Objective: 1.To understand the clinical characteristics of acne vulgaris with sensitive skin and the cognition of acne vulgaris patients to sensitivity skin and functional skin care products,explore the related influencing factors of acne vulgaris with sensitive skin.2.To analyze the ultrasonic characteristics of acne vulgaris lesions,explore the application value of high frequency ultrasound in the objective assessment of acne vulgaris severity and acne vulgaris with sensitive skin.Methods:Part Ⅰ: A self-designed questionnaire was used to investigate acne vulgaris patients in the outpatient Department of Dermatology and venereology,the First Hospital of Lanzhou University from February 2022 to June 2022.The main contents of the questionnaire include the patient’s sociodemographic characteristics,disease characteristics,skin care habits,diet habits,exercise and rest habits,as well as the "Huaxi Questionnaire" scale to assess whether the face of acne vulgaris patients is sensitive skin,etc.After the investigation,the data were sorted out and statistically analyzed.Part Ⅱ: The 20 MHz ultrasound combined with power doppler flow imaging technology is used to examine the most severe target lesions on the face of acne vulgaris patients,observe and analyze their morphological and blood flow characteristics,discuss the application value of high-frequency ultrasound in objective assessment of the severity of acne vulgaris.The 20 MHz ultrasound combined with shear wave elastography technology is used to examine the non-lesions skin in the middle of the forehead,right cheek and chin of acne vulgaris patients.According to the "Huaxi Questionnaire",the patients are divided into acne vulgaris with sensitive skin group and acne without sensitive skin group.To explore the application value of high-frequency ultrasound in the objective assessment of acne vulgaris with sensitive skin by comparing the skin thickness and elastic characteristics of the two groups.Results:Part Ⅰ: 1.A total of 435 patients with acne vulgaris received questionnaires,and396 valid questionnaires were included in the final analysis.2.There were 222(56.06%)of 396 patients with acne vulgaris were accompanied by sensitive skin.Among 222 patients with acne vulgaris with sensitive skin,65.32% were mildly sensitive,34.23% were moderately sensitive,0.45% were severely sensitive,59.91%patients’ sensitive symptoms are secondary to acne vulgaris,21.17% patients’ sensitivity symptoms occur before acne vulgaris,and 18.92% patients’ sensitivity symptoms are concurrent with acne vulgaris skin lesions.3.Among 396 patients with acne vulgaris,only 35.61% knew about the sensitive skin,33.33% knew about functional skin care products,only 28.54% used soothing skin care products,37.12%used skin barrier repair skin care products.4.Binary Logistic regression analysis showed that the patient’s education background,type of skin light reaction,whether he suffered from allergic diseases,whether his parents suffered from acne,whether he frequently changed skin care products,the frequency of daily exposure to polluted environment,the frequency of UV protection and hours of intense physical activity per week were independent influencing factors of acne vulgaris accompanied by sensitive skin.Part Ⅱ: 1.A total of 33 target lesions were examined on the face of 33 patients with acne vulgaris,including 4 pimples,19 papules,4 pustules,3 nodules,and 3 cysts.Morphological characteristics of these lesions included small hypoechoic lesions above the dermis,focal hypoechoic/mixed echoic structures with length <5mm,and low/mixed echoic structures with length ≥5mm.In addition,hypoechoic zonal sinus tract and strong echoic calcification foci may be present in some lesions.2.There were significant differences in the characteristics of blood grading and distribution in different clinical types of acne lesions(P<0.05).And there were significant differences in the characteristics of blood grading in different ultrasonic types of acne lesions(P<0.05).3.Kappa value of consistency analysis between high-frequency ultrasound assessment and clinical assessment of acne vulgaris severity was 0.704,P<0.05.4.A total of 14 patients with acne vulgaris with sensitive skin and 15 patients with acne vulgaris without sensitive skin were examined with no-lesions skin in the middle of the forehead,right cheek and chin.There were some differences in skin thickness,Emean,Emin and Emax between two group,but the differences were not statistically significant(P>0.05).The SD at the forehead was different and the difference was statistically significant(P<0.05).Conclusions:Part Ⅰ: 1.The prevalence rate of acne vulgaris with sensitive skin is relatively high in Gansu region.Most of the sensitive symptoms of patients with acne vulgaris with sensitive skin are secondary to acne vulgaris and the severity is relatively mild.2.Patients with acne vulgaris have a low awareness of sensitive skin and functional skin care products,and the use rate of functional skin care products such as soothing and repairing skin barrier is low.3.The patient’s education background,type of skin light reaction,whether he suffers from allergic diseases,whether his parents suffer from acne,whether he frequently changes skin care products,the frequency of daily exposure to polluted environment,the frequency of UV protection and hours of intense physical activity per week may be related to the occurrence of acne vulgaris with sensitive skin.Part Ⅱ:1.The same type of acne vulgaris lesions can show different ultrasonic image characteristics,and different types of acne vulgaris lesions can also show the same ultrasonic image characteristics.2.High-frequency ultrasound combined with power doppler flow imaging technology to observe the blood flow characteristics of acne vulgaris lesions can provide valuable information for the assessment of inflammation status of lesions.3.High-frequency ultrasound has high application value in objective assessment of the severity of acne vulgaris.4.High-frequency ultrasound combined with shear wave elastography technology can quantitatively detect skin elastic characteristics,which is expected to be applied to the objective assessment of acne vulgaris with sensitive skin. |
