| Objective:1.To evaluate the clinical efficacy of intranasal acupuncture in the treatment of allergic rhinitis.2.To investigate the effect of intranasal acupuncture on the abundance and diversity of nasal flora in patients with AR and the mechanism of action in the treatment of AR.Methods:A total of 40 patients who met the inclusion criteria for AR were selected and randomly divided into 20 cases in the test group and 20 cases in the control group. In the experimental group, acupuncture was applied to both internal Yingxiang acupoints and nasal mound, using the method of flat tonic and flat diarrhea, and the needles were retained for 20 min, once a day,for a treatment course of 14 days. The control group took oral loratadine tablets,1 tablet(10mg)at bedtime, once a day for 14 days. The patients’ main symptom rating scale(TNSS),concomitant symptom rating scale(TNNSS,VAS)were recorded before and after treatment,as well as changes in microflora including species composition, diversity, keystone species analysis,KEGG by applying the method introduced by Watelet to collect nasal secretions, which were sequenced by 16SrDNA high-throughput technology.the two groups were compared and efficacy analysis was performed. This subject was statistically analysed using SPSS 26.0 software.Results:1.The TNSS,TNNSS and VAS scores of patients in both groups, after treatment, decreased compared with those before treatment, and the difference was statistically significant(P<0.05).In the comparison between groups, the test group was significantly better than the control group in the TNSS and VAS scores(P<0.05),while the TNNSS was not statistically significant.2.After treatment in both groups, the total effective rate was observed to be 85%in the test group and 70%in the control group. The effect of intranasal needling was significantly better than that of the control group, and the difference was statistically significant(P<0.05).3.The abundance of nasal flora was changed after treatment in both groups, and the test group had significantly higher genus of Aspergillus,γ-Aspergillus,Enterobacteriaceae,Enterobacteriaceae,Burkholderia and Immobacterium than the control group, the difference was statistically significant(P<0.05).4.There was no significant change in the structural diversity of species within the two groups before and after treatment, which was not statistically significant(P>0.05).There was a statistically significant change in the structural diversity of species between the two groups after treatment(P<0.05).5.Multiple metabolic pathways were predicted after treatment in both groups, which was statistically significant(P<0.05).Conclusion:1.Intranasal acupuncture can effectively improve the main symptoms such as nasal itching and sneezing, and concomitant symptoms such as headache, lacrimation and itchy eyes caused by rhinitis in AR patients, all of which are significantly better than oral loratadine tablets.2.Intranasal needing can modulate the levels of Aspergillus phylum,Gamma-Amastigotes,Enterobacteriaceae,Enterobacteriaceae,Burkholderia,Immobilia spp.and improve AR symptoms;and can modulate the div ersity of species.3.Intranasal needling can affect metabolic pathways to alleviate AR symptoms. |
