| Objective:To clarify the connotation and characteristics of the competency of online appointment nurses,and construct a scientific,applicable and practical online appointment nurses training course system based on the training objectives and the actual door-to-door service,so as to provide a reference for "Internet plus nursing service" to train door-to-door service specialists.Methods:1 The construction of online appointment nurse training course system.Based on the theory of job competency,literature review,policy research,semi-structured interview and questionnaire survey were used to preliminarily establish the training course system of online appointment nurses.The Delphi method was used to modify and improve the indicators of online nurse training course after two rounds of expert consultation questionnaires,and finally formed the online appointment nurse training course system.2 Preliminary application of the training course system for online appointment nurses.The contents of the course were set up according to the three-level indicators of the online appointment nurse training course system and the training period,training hours,training methods and assessment methods were determined by literature review,questionnaire survey results and group discussion.According to the admission criteria of online appointment nurse training objects,to nurses were selected to participate in the training.The training period is 10 weeks,with a total of 160 hours,including 80 hours of theoretical teaching,40 hours of practical ability training,and 40 hours of door-to-door service practice.The first 6weeks for theoretical teaching and practical skills to cross,after 4 weeks for door-to-door service practice.Before and after the training,theoretical knowledge and practical ability assessment,core ability evaluation and service quality satisfaction survey of door-to-door service objects were conducted.In order to evaluate the effectiveness of training course,a comparative analysis was conducted to evaluate the effect of training course.In this study,the pre-and post-training evaluation indexes were compared and analyzed by the pre-and post-training controlled trial.Results:1 The results of semi-structured interview: four topics were extracted:(1)strengthening comprehensive ability training;(2)strengthening targeted training of online appointment services;(3)strengthening multi-department cooperation training;(4)expecting diversification of the forms of training assessment.2 The results of questionnaire: 344 nurses were investigated on the status quo of on-the-job training and the needs of training contents,The implementation rate of pre-and in-service training,qualification certification,training of teachers,training methods and assessment for online appointment nurses was 2.3%~47.7%,while the demand rate was 16.3%~92.7%.There was insufficient training implementation and high demand.Among the 43 items in the training content questionnaire,the average score was distributed between 3.85 ± 0.88 points and 4.25 ± 0.76 points.The first 3points were the transportation of critical patients,knowledge of nursing psychology,identification and referral of patients with acute myocardial infarction,cerebral infarction and cerebral hemorrhage.3 The results of expert letter inquiry: 18 experts participated in the inquiry,the two rounds of expert inquiry questionnaire recovery rate were 100%,94.44%,respectively,the expert authority coefficient was 0.928,0.938,respectively;Kendall’s W was 0.185 and 0.284,respectively,and the average score of each index was 4.06-5,and the coefficient of variation was 0-0.15.The training course system of online appointment nurses consists of three parts: including the access standard of online appointment nurse training objects,online appointment nurse training objectives,and the content of online appointment nurse training courses.Among them,the training course of online appointment for nurses includes 4 first-level indicators,19 second-level indicators and 58 third-level indicators.4 The results of course application: In this study,28 nurses were selected to participate in the online nurse appointment training,with an average age of 35.54±5.00 years and an average working life of 13.64±5.97 years.Four of them quit the training due to personal reasons.The theoretical assessment scores before and after the training were 69.34±7.23,81.83±5.62,respectively,and the scores of practical ability assessment were 74.69±4.90 and 85.35±2.62,respectively;the total core ability score was 180.54±12.61 and 218.54±33.84,respectively;and the satisfaction degree of service quality satisfaction of the clients were improved and P <0.05,the difference was statistically significant.Conclusion:1 The post training of online appointment nurses is insufficient,and the training demand is high.2 Based on the theory of post competency,the contents of the online appointment nurse training course system were constructed,and combined with the survey results of the job training needs of online appointment nurses,with the key competency characteristics required by online appointment nurses as the core knowledge points.The training methods are various,and it is reliable and scientific.3 Through the practical test of online appointment nurse training course system,the study shows that the training course system based on post competence can effectively improve the online appointment nurse home nursing ability,comprehensive practice ability and the service quality satisfaction degree of the clients,it has certain clinical reference and application value,and can provide reference basis for “Internet +Nursing Service” to train door-to-door service specialist talents. |
