| Objective: To evaluate the subjective visual quality of patients after ICL-V4 c implantation,and to analyze the factors affecting the visual quality,and to investigate the possible factors of the Ring-shaped dysphotopsia(halo)specific to ICL-V4 c lens.Methods:Forty-eight patients who underwent ICL-V4 c implantation in the dominant eye were included and followed up from 1 day after surgery to 3 months after surgery.Patients were examined for visual acuity,IOP,QIRC,halo visual performance,central hole central location(The ICL-V4 c central hole location was determined with respect to the pupil center and corneal apex),vault,halo size,number and position checked.Fixed ambient luminance,classified as central,complex,non-central and no-halo by halo location,was recorded.Results: The statistical results of patients at 3 months postoperatively were between ±0.50 D SE in 83.3% and between ±1.00 D SE in 96% of patients.The difference between SE at 1 month postoperatively(0.14 ± 0.49 D)and at 3 months postoperatively(-0.21±0.40 D)was not statistically significant(P=0.258).The actual corrected SE at 3months postoperatively was positively correlated with the preoperative pre-corrected SE values(P<0.001),presenting good validity,stability and predictability.All patients had postoperative halos under fixed environmental conditions that varied with the point source and the halos seen in binoculars were the sum of monoculars.At follow-up,there was a tendency for patients to recover their halos.We observed no significant change in the maximum halo diameter between 1 week and 1 month postoperatively(P=0.263);while the rest of the different time periods showed significant differences(P<0.05).At 1 day postoperatively,K1 and K2 were negatively correlated with the diameter of the postoperative maximal halo(P<0.05),and the horizontal displacement Cx of the center of the central hole relative to the corneal apex and the horizontal displacement Px of the center of the central hole relative to the pupil center were positively correlated with the diameter of the maximal halo(P<0.05).The QIRC scores of all patients were statistically different(P=0.000)from preoperative(38.21±3.84)compared to 3 months postoperative(43.56±5.21),especially in visual and physical function and social activity(P<0.05),in addition,the degree of distress of the halo at 3 months postoperative was negatively correlated with the psychosomatic scores of QIRC(P=0.003),and there was a difference in the severity and frequency of the halo between the complex and non-halo type(P<0.05).Conclusion: The quality of vision is significantly higher after ICL-V4 c implantation,but the Hole ICL-evoked ring-shaped dysphotopsia can cause problems for patients in the early postoperative period. |
