| Purpose: By investigating the status quo of self-assessment of health and utilization of non-medical health services among elderly chronic disease groups in Urumqi,the binary Logistic regression model was built based on the theoretical analysis framework of Anderson model to analyze the influencing factors of the elderly group’s utilization of non-medical health services in Urumqi.Using Propensity Score Matching(PSM)to explore the effect of the elderly chronic disease group using non-medical health services,to provide guidance and suggestions for the elderly chronic disease group to maximize the use of non-medical health services.Methods: Questionnaire was designed by pre-survey and group discussion.Simple random sampling and convenient sampling were used to select community residents in 8 districts of Urumqi city and distribute questionnaires.The research tools were designed and classified based on the Anderson model theoretical framework,including propensity factors(gender,age,ethnicity,occupation,etc.),enabling factors(living conditions,number of visits by children,main sources of income,ways of supporting the elderly,etc.)and demand factors(types of chronic diseases,scales of daily living ability,etc.).The self-rated health status of the elderly was obtained by using the self-rated daily living ability scale and its effect was studied.SPSS26.0 and Stata15.0statistical software were used for descriptive statistics,Chi-square test,rank sum test and binary Logistic regression analysis.Results: 1.The rate of health impairment in the elderly population with chronic diseases was 58.6%,the utilization rate of health management service was 93.6%,health security and financial service 86.8%,health product service 84.9%,health promotion service 66.7%,intelligent health technology service 35.6%,and health-related service 11.3% in non-medical and health services.2.The main factors affecting the use of non-medical health services by elderly chronic disease groups include residence,education level,pre-retirement occupation,frequency of meeting with children,family support,type of chronic disease,duration of chronic disease(P<0.05),etc.Propensity factors,enablement factors and need factors all have significant influence on the utilization of non-medical health services by the elderly chronic disease groups.Except for the health-related services which are most affected by demand factors,the utilization of other non-medical health services is most affected by enablement factors.Among them,the understanding and need of non-medical health services are common influencing factors for the elderly chronic disease groups to use non-medical health services.3.The results of this study showed that the health status of elderly chronic disease groups was affected by residence location,age,education level and other factors(P<0.05).PSM analysis showed that the use of health promotion services and health-related services was related to the health status of the elderly chronic disease group,among which health promotion services(ATT=0.19,P<0.05)and health-related services(ATT=0.21,P<0.05)had a positive impact on the physical status of the elderly chronic disease group.Health management service(ATT=0.09,P>0.05),health assurance and financial service(ATT=0.02,P>0.05),intelligent health and technical service(ATT=0.07,P>0.05),health product service(ATT=0.03,P>0.05)were not correlated with the health status of the elderly chronic disease group.Conclusion: The elderly population with chronic diseases in Urumqi suffers from frequent health problems,and their health status is not optimistic.The health status of senile chronic disease groups is affected by many aspects,and there are great differences among different characteristic groups.Health promotion service has positive effect on the health status of elderly chronic disease groups.It is suggested to pay attention to the elderly,living alone,poor economic status,and the coexistence of multiple diseases,actively carry out chronic disease prevention and treatment,focus on intervention of high-risk groups,advocate a healthy lifestyle,encourage social forces to participate in the supply of health services for the elderly chronic disease groups,promote the design of health services suitable for aging,improve the professional level of personnel,improve the quality of service,enrich the ways of health education,and promote the utilization of services. |
