Objective: To compare the efficacy and safety of linaclotide combined with polyethylene glycol and senna leaf combined with polyethylene glycol in bowel preparation for patients with constipation. Methods: A total of 180 patients with constipation who underwent colonoscopy in our hospital from October 2020 to May 2022 were selected and randomly divided into group A,B and C,with 60 cases in each group. Before the colonoscopy.Patients in group A received linaclotide +2L polyethylene glycol.Patients in group B were given senna granules +2L polyethylene glycol;Patients in group C received 3L polyethylene glycol,of which group C was the control group. The indexes of intestinal cleanliness, incidence of adverse reactions(abdominal pain, abdominal distension, nausea and vomiting),intention of re-bowel preparation willing and sleep quality were observed in the three groups. The indexes related to intestinal cleanliness included: BBPS score of bowel(left, middle, right colon and summary bowel);Colonoscopy time(entry time, withdrawal time, total time); The number of defecation; The time of first defecation;Polyp detection rate(PDR);Adenoma detection rate(ADR);The rate of cecal intubation. Results: A total of 180 patients who met the inclusion and exclusion criteria were enrolled in this study and divided into three groups(A,B and C)in a 1:1:1 ratio. In group A,colonoscopy was terminated in 1 patient because of not taking medicine as required and 1 patient because of rectal tumor. One patient in group B cancelled colonoscopy because of severe abdominal pain after medication.Three patients in group C cancelled colonoscopy because of poor bowel preparation.Finally,58 patients in group A,59 patients in group B and 57 patients in group C were enrolled in this study.(1)The BBPS scores of each intestinal segment and total intestinal segment in groups A and B were better than those in group C(P<0.05).The BBPS scores of each intestinal segment and total intestinal segment in group A were better than those in group B(P<0.05).(2)The entry time, withdrawal time and total colonoscopy time in group A and group B were lower than those in group C(P<0.05),and the withdrawal time, withdrawal time and total colonoscopy time in group A were lower than those in group B(P<0.05).(3)The first defecation time and defecation frequency in group A and group B were lower than those in group C(P<0.05).The first defecation time and defecation frequency in group A were lower than those in group B(P<0.05).(4)There were no significant differences in polyp detection rate, adenoma detection rate and cecal intubation rate among the three groups(P>0.05).(5)There was no significant difference in the incidence of abdominal pain among the three groups(P>0.05).The incidence of abdominal distension in group A and group B was lower than that in group C,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of abdominal distension between group A and group B(P>0.05).The incidence of nausea in group A was lower than that in group B and group C(P<0.05).There was no significant difference in the incidence of nausea between group B and group C(P>0.05).(6)The sleep quality and willingness of repeat bowel preparation in group A were higher than those in group B and group C(P<0.05).There was no significant difference in sleep quality and willingness of repeat bowel preparation between group B and group C(P>0.05).Conclusions: Linaclotide combined with polyethylene glycol and senna leaf combined with polyethylene glycol have advantages in improving the efficacy and safety of bowel preparation in patients with constipation.Linaclotide combined with polyethylene glycol has more advantages in improving the intestinal cleanliness of patients with constipation, which can significantly improve the quality of intestinal preparation and reduce the incidence of adverse reactions. It is a more optimized intestinal cleaning scheme.
