Objective:To investigate the correlation between the serum levels of N-telencephalic natriuretic peptide precursor(NT-pro BNP),troponin,red blood cell distribution width(RDW),neutrophil lymphocyte ratio(NLR),D-dimer,platelet lymphocyte ratio(PLR),and pulmonary artery pressure in patients with pulmonary embolism(pulmonary embolism,PE).Methods:Selected from December 2016 to December 2022 in the first affiliated hospital of Xinjiang medical university,a total of 343 cases of PE patients with lower extremity deep vein thrombosis,related hematology indexes,deep venous ultrasound of pulmonary artery systolic pressure,heart ultrasound of right heart,to explore the clinical application value of lower extremity deep vein thrombosis and hematology index.Results:Serum NT-pro BNP,troponin positive rate,RDW,NLR,right ventricular outflow tract width,right chamber diameter,and pulmonary artery width were significantly higher than the PE alone group,and the difference was statistically significant(P <0.05).In patients with PE and PH,the rate of severe PH was higher in the lower extremity than in the lower extremity,with a statistically significant difference(P<0.05).After Spearman correlation analysis,NT-pro BNP,RDW,and troponin positive showed significant correlation with right ventricular outflow tract width,right chamber diameter,main pulmonary artery width(P <0.05),all had positive correlation,and NT-pro BNP and pulmonary artery systolic pressure had positive correlation.Multivariate logistic regression analysis showed that serum NT-pro BNP,troponin positive,RDW,and NLR affected PH in PE patients and were their independent risk factors,while the presence of distal lower extremity DVT was an independent risk factor for moderate to severe PH in PE patients.In the ROC curve analysis,the ROC curves of the four hematology indexes were larger than those of the ROC curve was 0.771,the sensitivity was 65.8% and the specificity was 81.4%.The sensitivity had no significant change compared with the detection alone,but the specificity was significantly improved.Conclusion:In patients with PE and PH,NT-pro BNP,troponin positive rate,RDW,and NLR levels were increased,and showed a positive correlation with right ventricular outflow tract width,right chamber diameter,and main pulmonary artery width,which can be used as a reference index to evaluate the right heart condition in patients with PE.NT-pro BNP,RDW,NLR,and troponin positivity were associated with PH in PE patients and were independent risk factors for PH in PE patients,and the combination of all four could improve the predictive value of their separate detection.Furthermore,the presence of distal deep vein thrombosis in the lower extremity was an independent risk factor for PE patients with moderate to severe PH.
