Relationship Between Serum Cystatin C Levels And Central Arterial Pressure And Pulse Wave Conduction Velocity In Patients With Essential Hypertension

Objective:To investigate the correlation between serum cystatin C and central arterial pressure(CAP)and pulse wave conduction velocity(baPWV)in patients with essential hypertension.Method:A total of 599 patients with essential hypertension diagnosed in the Department of Hypertension,the First Affiliated Hospital of Xinjiang Medical University from January 2019 to October 2020 were selected.Serum cystatin C was detected and the patients were divided into low serum cystatin C group(153 males and 159 females)and high serum cystatin C group(150 males,137 cases in women).General data of the patients were collected and central arterial pressure and Doppler flow maps of the limbs were measured.Results:General data showed no significant differences in age,sex,BMI,systolic blood pressure,diastolic blood pressure,heart rate,fasting blood glucose and LDL-C between the two groups(P>0.05).There were statistically significant differences in height,weight,triglyceride,total cholesterol,HDL-C,creatinine(CR),urea nitrogen,uric acid and baPWV(P<0.05).Pearson correlation analysis showed that serum cystatin C was significantly correlated with baPWV brachial and ankle pulse wave conduction velocity(left),baPWV brachial and ankle pulse wave conduction velocity(right),CSP central artery systolic pressure,CDP central artery diastolic pressure,AP overflow pressure,and CPP central pulse pressure(P<0.05),and a positive correlation.Linear stepwise regression analysis showed that triglyceride and Cys-C were risk factors for elevated CSP.[β=1.37,P=0.011;β=0.013,P<0.001].Cys-C is a risk factor for CDP elevation.[β=-0.058,P=0.003].HDL-C,Cys-C and uric acid were risk factors for CPP elevation[β=2.877,P=0.001;β=0.052,P=0.009;β=-0.013,P=0.021].Creatinine(CR),Cys-C,HDL-C and uric acid were risk factors for AP elevation[β=2.877,P=0.001;β=2.877,P=0.001;β=2.877,P=0.001;β=2.877,P=0.001;β=-0.016,P<0.001].Cys-C is a risk factor for elevated baPWV.[β=59.726,P=0.015].Conclusion:Serum cystatin C in patients with essential hypertension is significantly correlated with CSP,CDP,CPP,AP and baPWV.The detection of the three indexes of central arterial pressure,cystatin C and baPWV has auxiliary diagnostic significance in the diagnosis of essential hypertension and the judgment of target organ damage.It provides important help for the treatment management and prognosis evaluation of patients with essential hypertension.
