Objective: To analyze the relationship between serum uric acid(UA)and homocysteine(Hcy)and the risk of stroke,and to explore whether there is a joint effect of serum UA and Hcy on the incidence of stroke.Methods: This study is a cross-sectional survey,with data from the Northeast China Rural Cardiovascular Health Research Project conducted from July 2012 to August 2013.The project included 11956 residents aged 35 and above as the research subjects.On this basis,7212 subjects were selected and finally included in this study.Use Epi Data v3.1 to enter the collected data and establish a database,and use SPSS 26.0 statistical software for statistical analysis.Demographic characteristics and various blood indicators were statistically described by means of mean and its standard deviation,median and interquartile spacing or composition comparison.Student’s t-test,analysis of variance,chi-square test and Wilcoxon rank sum test were used for inter-group analysis.The unconditional Logistic regression model was used to establish a multifactor adjustment model to analyze the relationship between serum UA and Hcy and the risk of stroke.At the same time,age,sex,nationality,education,smoking history,drinking history,and family history of hypertension were used as stratified variables to analyze the correlation between serum UA and Hcy and the incidence of stroke.All subjects were divided into four subgroups according to the level of serum UA and Hcy,namely,normal UA/normal Hcy group,normal UA/high Hcy group,high UA/normal Hcy group,and high UA/high Hcy group.The incidence risk of four subgroups and the multiplicative interaction of serum UA and Hcy were calculated in the Logistic regression model,and the subtypes of stroke were analyzed respectively to evaluate whether there was a joint effect of different levels of serum UA and Hcy on the risk of stroke.Results: 1.Among the study population,there were 703 stroke patients.The study subjects were divided into two groups according to whether stroke occurred or not,and the relevant characteristics were compared.The average age,systolic blood pressure(SBP),diastolic blood pressure(DBP),resting heart rate(HR),body mass index(BMI),waist circumference(WC),fasting blood glucose(FPG),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL_C),UA and Hcy levels of stroke group were higher than those of non-stroke group;There was no statistically significant difference between the two groups in gender,nationality and smoking history.2.After multifactor adjustment,the risk of stroke and ischemic stroke in the high UA group was 1.612 times(OR: 1.612,95% CI: 1.288-2.017)and 1.492 times(OR: 1.492,95% CI: 1.074-2.072)of that in the normal group,and the risk of stroke and ischemic stroke in the high Hcy group was 1.235 times(OR: 1.235,95% CI: 1.035-1.474)and1.731 times(OR: 1.731,95% CI: 1.317-2.275)of that in the normal group,However,the high UA group and high Hcy group had no statistical significance for the risk of hemorrhagic stroke.3.The relationship between the combined action of different levels of UA and Hcy and the risk of stroke and its subtypes was analyzed by Logistic regression model.It was found that the risk of stroke in the high UA/high Hcy group increased by 1.771 times(OR: 1.771,95% CI: 1.320-2.326,P<0.001),and the risk of ischemic stroke increased by 2.551 times(OR: 2.551,95% CI: 1.689-3.854,P<0.001),taking the normal UA/normal Hcy group as a reference,There was no statistical significance in the risk of hemorrhagic stroke.When high UA and high Hcy exist at the same time,the risk of stroke and ischemic stroke is higher than that when they act alone,especially in ischemic stroke.Interaction multiplication model(high UA × High Hcy group)had no statistical significance in stroke,ischemic stroke and hemorrhagic stroke.Conclusion: 1.There is a positive correlation between serum UA and Hcy and the risk of stroke and ischemic stroke.2.When serum UA and Hcy are at high levels at the same time,the risk of stroke and ischemic stroke is higher than that when they are at high levels alone,and the role in ischemic stroke is more significant.
