Ten-Year Survival Rate And Influencing Factors Of Urban Thyroid Cancer Patients In Liaoning Province

Objective:Thyroid cancer is a relatively common endocrine malignancy,and it is also one of the fastest-growing malignant tumors in our country,which has brought heavy health pressure and disease burden to China.Through this study,we analyzed the 10-year survival rate of patients with thyroid cancer in cities in Liaoning Province,and explored various factors affecting their prognosis.It fills in the vacancy of 10-year survival research of patients with thyroid cancer in Liaoning,and enriches the survival research of thyroid cancer in Liaoning province.Methods:Our team established a population cohort in 2002,and selected all urban thyroid cancer patients diagnosed in Shenyang,Anshan and Benxi from the tumor incidence report database of Liaoning Province respectively(diagnosis time: 2000.01.01-2002.12.31)and a follow-up survey was conducted,and then the personal and clinical information of thyroid cancer patients was retrospectively investigated through the self-compiled follow-up list of diagnosis,treatment and survival status.The follow-up date of survival status was December31,2012.SPSS26.0 software was used to analyze the data.The observed survival rate(OSR)was calculated by Kaplan-Meier method,and the expected survival rate(ESR)was calculated by Ederer Ⅱ method.The relative survival rate(RSR)was the ratio of OSR to ESR.Kaplan-Meier method was used to draw the survival curve,and Cox proportional risk regression model was used to analyze the single and multi-factor factors affecting the prognosis of thyroid cancer patients.Results:A total of 178 patients with thyroid cancer in three cities in Liaoning Province were included in this study,and the ten-year OSR and RSR were: 70.79% and 76%,respectively.The ten-year ORS and RSR of urban thyroid cancer patients in Liaoning province were :61.76% and 71.15% for male patients and :72.72% and 77.18% for female patients.ten-year OSR and RSR of patients aged 0-64 years were 85.31% and 89.35%,respectively,and ten-year OSR and RSR of patients aged 65 years and above were : 11.43%,16.71%.The10-year OSR and RSR for patients with stage I-II thyroid cancer were: 85.37% and 91.50%,respectively;for patients with stage III-IV cancer,the 10-year OSR and RSR were: 43.48%and 48.31%,respectively;and for patients with unknown clinical stage,the 10-year OSR and RSR were: 72% and 74.43%,respectively.The 10-year OSR and RSR for patients treated with surgical modalities were: 81.76% and 87.24%,respectively,while the 10-year OSR and RSR for patients not treated with surgical modalities were: 5.26% and 6.22%,respectively,and the10-year OSR and RSR for patients with unknown surgical modalities were: 36.36% and38.43%,respectively.The ten-year OSR and RSR for differentiated patients were: 84.38% and89.29%,respectively;for undifferentiated patients: 14.29% and 15.03%,respectively,and for medullary patients: 100%;for patients with unknown tissue typing: 39.22% and 44.02%,respectively.The results of the comparison of follow-up outcomes and survival curves all showed statistically significant differences in survival curves for age,clinical stage,surgery or not,and tissue typing.The results of the one-way Cox analysis showed that age,clinical stage,surgery or not,and tissue staging were independent factors affecting the prognosis of thyroid cancer.Multi-factor Cox analysis showed that age,surgery or not,and tissue staging were the main factors influencing the prognosis of thyroid cancer patients.Conclusion : In this study,the 10-year survival rate of urban thyroid cancer patients in Liaoning province was studied,and various factors affecting the prognosis of patients were analyzed.The results showed that there was still a certain gap between the 10-year survival rate of urban thyroid cancer patients in Liaoning province and that of developed areas and countries.The results showed that age,clinical stage,surgical treatment or not,and tissue type were independent factors affecting the prognosis of thyroid cancer patients.
