Objective: The purpose of this study is to explore the construction of a core competency evaluation index system for elderly caregivers in combined medical and nursing institutions suitable for China’s national conditions.It provides a scientific basis for the training,assessment and evaluation of the core competencies of nursing staff in medical and nursing institutions,provides certain directions for the development of nursing staff in medical and nursing institutions,and provides some help to standardize the content of medical and nursing services and improve the quality of medical and nursing services.Methods: This study takes the core competency theory as the framework,and the initial entries of evaluation indexes are finally formulated through literature review,combining the current situation of the development of domestic elderly caregivers,querying the relevant national policy documents about elderly caregivers and health care integration institutions,as well as expert interviews and group discussions.Through pre-survey,the framework of entries for the Delphi method consultation was formed.After consulting using the Delphi method,the indicators were revised based on the results of 2 rounds of consulting by 20 experts to initially establish the core competency evaluation index system for elderly caregivers in combined health care institutions.Finally,using the hierarchical analysis method,we set the weights and consistency test for the constructed index system,and finally formed the core competency evaluation index system for elderly caregivers in combined medical and nursing institutions.Results:1.The core competency evaluation index system of elderly caregivers in combined medical and nursing institutions includes 6 primary indicators,17 secondary indicators and 59 tertiary indicators.2.The response rate and effective rate of the questionnaire in 2 rounds of this study were 100.00%.The rate of experts proposing revisions in the two rounds was 65.00%and 40.00%,respectively;the full score ratio in the second round ranged from 25.00%to 100.00%,the mean importance assignment was between 4.40 and 5.00,and the standard deviation was between 0.00 and 0.51.The coefficients of variation(CV)of the first-tier indicators in the second round were in the range of 0.00 to 0.17,and the CVs of the second-tier and third-tier indicators were in the range of 0.00 to 0.11.The coefficient of judgment(Ca)of the experts was calculated to be 0.97,the familiarity(Cs)was 0.76,and the coefficient of authority(Cr)was 0.86.The coefficient of coordination for the first round of consultation was 0.20,and the coefficient of coordination for the second round of consultation was 0.24,and the p-values were less than 0.05,and the differences were statistically significant.3.The weight values of the first-level indicators were interpersonal skills(0.1312),personal career and development skills(0.3180),professional ethical qualities(0.3180),critical thinking(0.0857),organizational management skills(0.0857),and education and training skills(0.0613).The weights of the primary indicators ranged from 0.0613 to 0.3180;the weights of the secondary indicators ranged from 0.0114 to0.2385.The Cr values of each judgment matrix ranged from 0.0000 to 0.0520.the Cr values of all hierarchical indicators were <0.1,which had satisfactory consistency and passed the consistency test,and the indicator weights were set reasonably.Conclusion: This study constructed an index system for evaluating the core competencies of elderly caregivers in medical and nursing institutions,including 6primary indicators,17 secondary indicators and 59 tertiary indicators.The positivity,authority and opinion coordination of expert consultation are good,and the opinions have a high degree of consistency.The weight values of indicators at all levels were determined by hierarchical analysis method with satisfactory consistency,and the weights of each indicator were set reasonably.Thus,it ensures that the constructed core competence evaluation indexes of elderly caregivers in combined medical and nursing institutions are reliable,scientific and practical,which can provide reference for the training,development,evaluation as well as assessment of elderly caregivers in combined medical and nursing institutions and promote the development of elderly care business in combined medical and nursing institutions.
