Construction Of Evaluation Index System Of Core Competence Of Gynecological Specialist Nurses

Objective: This study aims to explore the needs of different groups in the field of gynecology for the core competence of gynecological nurses,and build a scientific and reliable evaluation index system for core competence of gynecological specialist nurses.Methods: This study is a mixed study and is mainly divided into the following four parts.1.Literature review: Through reviewing the relevant literature of gynecological specialist nurses at home and abroad,the entries that conform to the framework of the evaluation index system of gynecological specialist nurses are extracted,and the entry pool of the core competency evaluation index system of gynecological specialist nurses is initially formed.2.Semi-structured interview: Through the objective sampling method,medical nursing experts,nurses and inpatients in gynecology wards were selected for semi-structured interviews,and the content analysis method was used to analyze the data,and the entry pool was further improved,and the first draft of the core competency evaluation index system of gynecological specialist nurses was formed.3.Delphi method: Two rounds of expert consultation were conducted for 15 experts who met the expert inclusion criteria by questionnaire star and email,and the statistics and analysis of data were carried out by Excel2019 and SPSS25.0 software,and the reliability and scientificity of the two rounds of expert consultation were tested according to the expert enthusiasm coefficient,expert authority,expert opinion concentration and expert opinion coordination.According to the two rounds of expert advice,the index system was revised and improved,the final draft of the core competency evaluation index system for gynecological specialist nurses was formed,and then the analytic hierarchy method was used to determine the weight of indicators at all levels.4.Reliability and validity test: Based on the core competency evaluation index system of gynecological specialist nurses,the self-assessment questionnaire of core competence of gynecological specialist nurses was formulated,and the questionnaire stars were distributed to the research subjects included in the standard by convenient sampling method,and the statistics and analysis of data were carried out by Excel2019 and SPSS25.0 software,and the reliability coefficient of Cronbach’s α was calculated,and the validity of its content and structure validity was calculated,so as to test the reliability and validity of the index system.Results:1.Through the literature review method,and based on the Harmic advanced practice nursing theory model,the items that meet the above dimensions were extracted.An item pool including clinical practice ability(30 items),professional development ability(8 items),communication and coordination ability(3 items),professional humanistic traits(5 items)and leadership ability(4 items)was preliminarily formed.2.In the semi-structured interview stage,10 patients,10 nurses,8 nursing managers and1 physician were interviewed,and the core competency evaluation index system of gynecological specialist nurses including 5 first-level indicators,15 second-level indicators and 62 third-level indicators was preliminarily formed through group discussion,which provided a basis for the next step of expert consultation.3.A total of 15 experts were included in this study,and the positive coefficients of experts in two rounds were 100% and 88.23%.The authoritative coefficients of the two rounds of experts were 0.795 and 0.880,and the Kendall coordination coefficients of the two rounds of experts were 0.176 and 0.241,both P<0.001.It shows that the expert is authoritative and has a good concentration of opinions;According to the consultation opinions of experts in the two rounds of expert consultation,the index system was modified,and the core competency evaluation index system of gynecological specialist nurses containing 5 first-level indicators,14 second-level indicators and 64 third-level indicators was formed,and the weight coefficient and consistency test results of the indicators at all levels were calculated by using the analytic hierarchy method,and the results showed that the weights of the indicators at all levels were reasonably formulated.4.In this study,a total of 61 gynecological specialist nurses were surveyed,and the results showed that the overall Cronbach’s α coefficient of the core competency evaluation index system of gynecological specialist nurses was 0.974,the validity of the content of the entries was concentrated in 0.833～1.000,the validity of the content of the scale was 0.976,the correlation coefficient between all levels of indicators was >0.4,and the P<0.001 was 0.001,indicating that the evaluation index system had good reliability and validity.Conclusion: This study finally formed a core competency evaluation index system for gynecological specialist nurses with 5 first-level indicators,14 second-level indicators and 64 third-level indicators with good reliability and validity,which provided guidance for the education and training,ability evaluation of gynecological specialist nurses in China.
