Study On The Sense Of Coherence,Care Burden And Care Ability Of The Main Caregivers Of Elderly Patients With Chronic Obstructive Pulmonary Disease

Objective:To investigate the status quo and influencing factors of sense of coherence,care burden and care ability of the main caregivers of elderly patients with chronic obstructive pulmonary disease,to study the relationship among the three,and to analyze the mediating role of sense of coherence between care burden and care ability,so as to provide theoretical basis for improving the mental health of the main caregivers,improving the level of sense of coherence,care ability and reducing the care burden of caregivers.Methods:From January 2022 to June 2022,300 patients and main caregivers who met the criteria in the ward of geriatrics,respiratory medicine and general practice in a thirdgrade general hospital in Neijiang City were selected by convenient sampling method for questionnaire survey.The survey tools include : general information questionnaire,psychological consistency scale,care burden scale and care ability scale.The statistical software SPSS 26.0 and SPSS PROCESS3.3 were used.The statistical methods mainly used descriptive statistical analysis,independent sample t test,one-way analysis of variance,Pearson correlation analysis,regression analysis and mediating effect analysis.Result:1.The results of the general information questionnaire showed that there were300 groups of patients and caregivers in the survey.The main caregivers were married272 people(90.7 %),196 women(63.5 %),and 116 spouses(38.7 %).Among them,108 people(36.0 %)had primary school education and below,140 people(46.7 %)had an average daily care time of 6-8 hours,and 181 people(60.3 %)had assisted caregivers.2.The total score of sense of coherence of the main caregivers of elderly patients with chronic obstructive pulmonary disease was(56.56 ± 10.00),which was at a low level.Multiple linear regression analysis of sense of coherence showed that the frequency of pulmonary rehabilitation training,the gender of caregivers,family monthly income,care burden and care ability could jointly explain 83.1 % of the variation of sense of coherence.3.The total score of care burden of primary caregivers of elderly patients with chronic obstructive pulmonary disease was(44.32 ± 11.83).Multiple linear regression analysis of care burden showed that the comprehensive assessment of COPD,self-care ability,economic burden of caregivers,sense of coherence and care ability could explain 64.2 % of the variation of care burden.4.The total score of caring ability of primary caregivers of elderly patients with chronic obstructive pulmonary disease was(27.59 ± 8.06),which was at a low level.Multiple linear regression analysis of care ability showed that the comprehensive assessment of COPD patients,the sense of coherence of caregivers and the burden of care could jointly explain 60.2 % of the variation of care ability.5.The results of correlation analysis between sense of coherence and care burden and care ability of main caregivers of elderly patients with chronic obstructive pulmonary disease showed that sense of coherence was significantly negatively correlated with care burden(r =-0.608,P < 0.001),sense of coherence was significantly positively correlated with care ability(r =-0.582,P < 0.001),and care burden was significantly negatively correlated with care ability(r = 0.730,P < 0.001).6.The mediating effect analysis showed that after controlling the covariates,the sense of coherence of the main caregivers of elderly patients with chronic obstructive pulmonary disease played a partial mediating role in their care burden and care ability,and the mediating effect value was 19.23 %.Conclusion:1.The main caregivers of elderly patients with chronic obstructive pulmonary disease are psychologically consistent,the care ability is at a low level,and the care burden is high.2.The sense of coherence of the main caregivers of elderly patients with chronic obstructive pulmonary disease was affected by the frequency of rehabilitation training and the gender,family monthly income,care burden and care ability of the caregivers.The care burden was affected by the comprehensive assessment of COPD,self-care ability and the economic burden,care ability and psychological consistency of the caregivers.The care ability was affected by the comprehensive assessment of COPD and the psychological consistency and care burden of the caregivers.3.As a mediating variable,sense of coherence can alleviate the reduction of care ability caused by care burden,suggesting that we should pay attention to the influence of positive psychology on care ability when formulating strategies to reduce patients ’care burden.
