| Purpose:To understand the status of psychological resilience,self-management and quality of life in patients with acute ischemic stroke(AIS).Analyze its influencing factors;to explore the correlation between the three;to explore the mediating effect of selfmanagement between psychological resilience and quality of life,and to provide basis and guidance for promoting the quality of life of patients.Materials and Methods:A cross-sectional survey method was used to conveniently select 236 AIS patients in the neurology ward of two tertiary hospitals in Chengdu from October 2021 to April 2022 as the research objects.A questionnaire survey was conducted on the subjects who met the inclusion and exclusion criteria by using the general data questionnaire,the psychological resilience scale,the stroke self-management ability scale and the health survey scale.EpiData 3.0,IBM SPSS Statistics 25.0 software and PROCESS V3.5 plugin were used for data entry and analysis.Descriptive analysis was performed on the basic characteristics of the subjects.Statistical analysis was performed using t test,analysis of variance,M-W test,H test,multiple regression analysis,Pearson/Spearman correlation analysis,sequential test method and Bootstrap.Test level(bilateral)α=0.05.Result:1.The psychological resilience score of patients with AIS was(24.33 ± 6.29).Multiple regression analysis of psychological resilience showed that family monthly income per capita,overall neurological function,widowhood and age could explain 40.50%of the variation of psychological resilience(F=41.015,P<0.05).2.The self-management score of AIS patients was(83.73±16.96),and the multiple regression analysis of self-management showed that monthly per capita household income,overall neurological functional status,age,education level,and medical payment method for NPS could explain 48.40%of the variance in self-management(F=45.061,P<0.05).3.The quality of life score of AIS patients was 70.45(56.82,81.77),68.88(46.06,81.75)in the field of physiological health,and 76.02(60.23,84.69)in the field of mental health.The results of multiple linear regression analysis showed that the overall neurological function status,per capita monthly income of families,and medical treatment payment method of the new rural cooperative medical system were important factors affecting physiological(F=210.499,P<0.05)and psychological quality of life(F=44.661,P<0.05).4.The results of correlation analysis showed that the psychological resilience of patients with AIS was significantly positively correlated with self-management and quality of life(P<0.01).5.The mesomeric effect analysis shows that the impact of resilience on the quality of life of AIS patients is through the mesomeric effect of self-management,and the mesomeric effect accounts for 30.17%of the total effect(β=0.296,95%CI=[0.3658,0.9364],P<0.001).Conclusion:AIS patients have low levels of psychological resilience and quality of life,and their self-management ability is at a moderate level.The higher the per capita monthly income of a family,the positive impact on the psychological resilience of AIS patients,while the overall neurological status,widowhood,and elderly age have a negative impact on the psychological resilience of AIS patients;The higher the per capita monthly income and educational level of the family,the positive impact on the self-management of AIS patients.The overall neurological function status is worse,the older the age,and the negative impact of the new rural cooperative medical system on the self-management of AIS patients;The higher the per capita monthly income of a family,the positive impact on the quality of life of AIS patients,the worse the overall neurological function,and the negative impact of the new rural cooperative medical payment method on the quality of life of AIS patients.The self-management of AIS patients has a mediating effect on the relationship between psychological resilience and quality of life. |
