Objective In this study,gadolinium contrast magnetic resonance imaging of the inner ear was used to observe endolymphatic hydrops of the inner ear,so as to explore the correlation between the degree of endolymphatic hydrops of the inner ear and clinical manifestations in patients with unilateral sudden deafness,in order to guide the treatment of patients with sudden deafness.Methods A total of 65 patients with unilateral sudden deafness diagnosed by pure tone audiometry,medical history investigation and other ear related examinations in the Department of Otolaryngology,Head and Neck Surgery,Jingzhou Hospital Affiliated to Yangtze University from October,2021 to January,2023 were included as study subjects.All patients received internal ear contrast MRI examination with gadolinium-based imaging within 3 days after admission.The patients were seated or supine and given a double dose(0.2 mmol/kg)of glumine gadolinium penate through the cubital vein for 4 hours,the imaging sequence was 3D fluid attenuated inversion recovery(3D FLAIR).Forty-seven patients with signs of endolymphatic hydrops on magnetic resonance images were screened out,and their general clinical data were collected,specifically:age,sex,side of disease,course of disease,number of days between onset and MRI with gadolinium in the inner ear,tinnitus,vertigo,mean threshold of hearing in the affected ear,classification of hearing curve for sudden deafness,degree of hearing loss,and degree of endolymphatic hydrops in the inner ear were recorded in the MRI images.Spearman rank correlation analysis was used to analyze whether the degree of endolymphatic hydrops in the inner ear of patients with unilateral sudden deafness was correlated with clinical manifestations and the degree of correlationResults In this study,different degrees of endolymphatic hydrops were found in the inner ear of 47 patients with unilateral sudden deafness,and the positive rate of development was 72.3%(47/65).In the analysis of correlation between the degree of endolymphatic hydrops in the inner ear of patients with unilateral sudden deafness and age,sex,disease side,tinnitus,vertigo and stuffy ear,P value was>0.05.In the correlation analysis with the course of disease and the days between the onset and the MRI examination of the inner ear,P value was<0.05 and r_s value was<0.In the correlation analysis with the degree of hearing loss and the mean hearing threshold of the affected ear,P value was<0.05 and r_s value was>0.Conclusion 1.There was no significant correlation between the degree of endolymphatic hydrops in the inner ear and the age,sex,side of the disease,tinnitus,vertigo,and stuffy ear of the patients with sudden deafness.2.The degree of endolymphatic hydrops in the inner ear was negatively correlated with the course of disease and the days between the onset and the MRI examination of gadolinium in the inner ear,that is,the longer the course of disease and the days between the onset and the MRI examination of the inner ear,the less severe the degree of endolymphatic hydrops;3.There is a positive correlation between the degree of endolymphatic hydrops in the inner ear and the degree of hearing loss and the mean hearing threshold of the affected ear,that is,the more serious the degree of hearing loss,the more serious the degree of endolymphatic hydrops in the inner ear.
