Objective: To explore the clinical efficacy of Jiawei Zhenwu decoction in treating chronic heart failure with yang deficiency and water generics,and to provide reference for the clinical treatment of this disease with traditional Chinese medicine.Methods: 66 patients with chronic heart failure of yang deficiency and water generalization type admitted from May 2020 to April 2021 in Min Qingcountry Hospital were selected as research objects.According to the envelope method,66 patients were divided into two groups according to the ratio of 1:1.After each group was written,the same opaque envelope was used to hide the scheme.Then,the envelopes were scrambled and coded,and every time a subject who met the inclusion criteria entered the study,the patients were numbered,and the envelopes with corresponding numbers were opened,and the intervention treatment was carried out according to the scheme of grouping in envelopes.The treatment scheme accepted by each subject is determined by the generated random sequence.The control group was treated with western medicine,and the test group was treated with modified Zhenwu decoction on the basis of the western medicine treatment scheme consistent with the control group.Comparison of therapeutic effects: before and after treatment,the N-terminal pro brain natriuretic peptide(NT pro BNP),hypersensitive C-reactive protein(hs CRP),left ventricular ejection fraction(LVEF),TCM syndrome score,cardiac function grading,and6-minute walking distance.To compare the treatment safety: before and after treatment,the liver function,alanine aminotransferase(ALT),aspartate aminotransferase(AST),renal function,creatinine(Cr),urea nitrogen(BUN),glomerular filtration rate(e GFR),urine quantitative protein,and the occurrence of adverse reactions,and the aggravation and new occurrence of the original arrhythmia.Results:(1)Baseline comparison: there were no significant differences in age,sex,NYHA heart function class,disease course,BMI,type of primary disease and type of complication between the two groups before treatment.(2)Treatment effect: The serum NT-pro BNP level,serum CRP level and TCM syndrome score of the two groups after treatment were lower than those before treatment(P<0.05),the LVEF after treatment was higher than that before treatment(P<0.05),and the 6-minute walking distance after treatment was longer than that before treatment(P<0.05).After treatment,the levels of serum NT-pro BNP,hs-CRP and TCM syndrome score in the experimental group were lower than those in the control group(P<0.05).(3)Safety of treatment: There was no statistical difference between the two groups in serum ALT level,serum AST level,serum Cr level,serum BUN level,e GFR and urine quantitative protein level after treatment(P>0.05),the incidence of adverse reactions between the two groups(P>0.05).The original arrhythmias of the two groups did not worsen,and no new arrhythmias occurred.Conclusion:(1)Applying modified Zhenwu Decoction to the patients with chronic heart failure caused by yang deficiency and water deficiency can effectively improve the clinical symptoms and signs of the patients,enhance cardiac function and improve activity tolerance.The effective rate of TCM symptom integral performance has also improved.(2)The use of modified Zhenwu Decoction in the treatment of chronic heart failure caused by yang deficiency and water is safe in short-term treatment.
