Objectives:By comparing the clinical efficacy of shallow acupuncture combined with massage and oral diclofenac sodium continuous release tablets in the treatment of chronic tension headache of liver qi stagnation, and the effects of the two methods on VAS pain score, headache symptom score and serum 5-HT content,it provides a certain reference value for the clinical treatment of chronic tension headache of liver qi stagnation. Methods:A total of 72 patients with chronic tension-type headache of liver qi stagnation were enrolled in this study. They were randomly divided into observation group(shallow acupuncture combined with massage group)and control group(oral diclofenac sodium sustained-release tablets group),36 cases in each group. The observation group was treated with massage for 20 minutes.Shallow needling was applied for another 30 min.The treatment was given once a day,7 days as a course of treatment, and a total of 1 course of treatment was given. For shallow acupuncture, the acupoints were Baihui,Fengchi(bilateral),Taiyang(bilateral),Hegu(bilateral)and Taichong(bilateral). The control group was treated with diclofenac sodium sustained-release tablets,75 mg each time, once a day,7 days as a course of treatment,a total of 1 course of treatment. The VAS pain score and headache symptom score before and after intervention and 1 month after the end of treatment, and the changes of serum 5-HT content before and after treatment were observed, and the curative effect was evaluated in the two groups.SPSS26.0 software was used to analyze the data.Rusults:1.A total of 72 patients were enrolled in this project, including 36 in the control group and 36 in the observation group.During the study period, one person in the observation group fell off(the treatment was suspended due to the impact of traffic),and two people in the control group fell off(the reasons were that they did not take medicine according to the requirements during the study period, trauma and fracture). The treatment of these falling off patients was less than half, and they were not included in the statistics. Therefore,a total of 69 people were finally included in the statistics,34 people in the control group and 35 people in the observation group.2.Before treatment, there was no significant difference in VAS pain score, headache symptom score and serum 5-HT content between the two groups(P>0.05).3.Comparison of VAS pain scores: after treatment, the VAS pain scores of the two groups were significantly lower than those before treatment(all P<0.05).The improvement degree of the observation group was better than that of the control group, and the difference was statistically significant(P<0.05).One month after treatment, the VAS pain score of the observation group was significantly lower than that of the control group(P<0.05).4.Comparison of headache symptom scores: after treatment, the headache symptom scores of the two groups were significantly lower than those before treatment(all P<0.05).The score improvement of the observation group was better than that of the control group, and the difference was statistically significant(P<0.05).After 1 month of treatment, the score of the observation group was significantly lower than that of the control group(P<0.05).5.Comparison of serum 5-HT content: after treatment, the serum 5-HT content of the two groups was higher than that before treatment(all P<0.05);The improvement of serum 5-HT content in the observation group was better than that in the control group, and the difference was statistically significant(P<0.05).6.Clinical efficacy evaluation: Clinical efficacy evaluation: in the total effective rate index, the observation group and the control group were 88.57% and 79.41%,respectively,the clinical efficacy of the two groups was statistically significant(P<0.05).One month after the end of treatment, the total effective rate of the treatment group was still 80.00%,which was significantly higher than 55.88% of the control group, and the difference was statistically significant(P<0.05).Conclusions:1.Shallow acupuncture combined with massage and oral diclofenac sodium sustained-r elease tablets can effectively reduce the VAS pain score and headache symptom score of patients with chronic tension headache caused by liver qi depression.2.Shallow acupuncture combined with massage and oral diclofenac sodium sustained r elease tablets can increase serum 5-HT level, which may be the potential mechanism of this therapy.3.The curative effect of shallow acupuncture combined with massage in the treatment of chronic tension-type headache of liver qi stagnation is better than oral diclofenac sodium sustained-release tablets, and has long-term curative effect.s.
