| Objective:To observe the changes of somatic symptoms,anxiety,depression and quality of life of discharged patients with advanced lung cancer of qi and Yin deficiency by implementing family-integrated acupoint massage continuous nursing intervention for one month.To explore the feasibility and effectiveness of family-integrated point massage in continuous nursing for patients with advanced lung cancer with deficiency of both qi and Yin.In order to reduce the physical and mental burden of patients,improve the quality of life of patients,further promote the application of point massage in home rehabilitation,and provide evidence-based data for home rehabilitation nursing of patients with advanced lung cancer.Methods:A randomized controlled trial design was used in this study.A total of 86 inpatients with advanced lung cancer of qi and Yin deficiency who were treated in a Class ⅲ Grade A hospital in Yunnan Province from November 2021 to July 2022 were selected and randomly divided into the control group and the intervention group,with 43 cases in each group.Ten cases were lost to follow-up,and 39 cases in the control group and 37 cases in the intervention group were finally selected.Patients in the control group received routine continuous nursing,while patients in the intervention group received family-integrated point massage on this basis,including the establishment of family-integrated continuous nursing cooperation group,member training,and family integrated continuous nursing guidance after discharge.The changes and differences in clinical symptoms,anxiety,depression,and quality of life before and 1 month after discharge were evaluated and compared between the two groups.A P value of less than 0.05 was considered to indicate statistical significance.Results:1.Comparison of Anderson symptom scores between the two groups(1)After intervention,the interaction effects of 9 symptom items of pain,fatigue,nausea,sleep disturbance,shortness of breath,poor appetite,sadness,vomiting and constipation and 4 symptom distress items of general activities,emotions,relationships with others and fun of life were significant between the two groups(P < 0.05),and the interaction effects of other items were not statistically significant(P > 0.05).(2)In the second,third and fourth week after intervention,there were differences in the scores of five symptom items such as fatigue and sadness and two symptom distress items such as general activity and emotion between the two groups(P < 0.05).At the third and fourth week after intervention,the scores of four symptom items such as pain and nausea and two symptom distress items such as relationship with others and joy of life were significantly different between the two groups(P < 0.05),and the intervention group was better than the control group,and there was no significant difference in the rest(P >0.05).2.Comparison of hospital anxiety and depression scores between the two groups(1)The within-group effect,between-group effect and interaction effect of anxiety and depression in the two groups were significant(P < 0.05).(2)In the second,third and fourth week after intervention,there were differences in anxiety scores between the two groups(P < 0.05).At the third and fourth weeks after intervention,the depression scores of the two groups were different(P < 0.05),and the scores of the intervention group were lower than those of the control group.3.Comparison of quality of life scores between the two groups(1)The interaction effect of the two groups on emotional function,physical function,social function,overall health status,nausea,pain,fatigue,shortness of breath,insomnia and appetite was significant(P < 0.05),and there was no significant difference in other items(P > 0.05).(2)There were significant differences in the scores of social function,emotional function,overall health status,fatigue,insomnia,shortness of breath and constipation between the two groups at the second,third and fourth week after intervention(P < 0.05).At the third and fourth week after intervention,the scores of physical function,nausea,pain and appetite were different between the two groups(P < 0.05),and the overall score of the intervention group was better than that of the control group.Conclusion:1.Family-integrated acupoint massage can effectively improve the pain,fatigue,nausea and other symptoms in advanced lung cancer patients with both qi and Yin deficiency,thereby improving their overall comfort.2.Family-integrated acupoint massage can effectively relieve anxiety and depression in advanced lung cancer patients with deficiency of both qi and Yin.3.Family-integrated acupoint massage can improve the the quality of life in patients with advanced lung cancer who have both Qi and Yin deficiencies. |
