Analysis Of The Pathogenesis Of Cervical Spondylopathy Of Radiculopathy And Observation Of The Therapeutic Effect Of Extensional Relaxation Therapy

Purpose:In this study,according to the pathogenesis and factors of radiculopathy,the principal component Logistic regression clinical prediction model was constructed to analyze the main pathogenesis factors of radiculopathy and the relationship among the influencing factors,so as to provide scientific thoughts and methods for the diagnosis,prevention,prognosis and rehabilitation of this disease.And on this basis,we included "equal emphasis on tendons and bones" as the treatment,"loosening tendons and regulating bones" as the treatment,to explore the clinical effect of its intervention in cervical spondylosis of nerve root type.Method:In the first part of the study,the data came from patients who had symptoms of cervical spondylosis and volunteered to fill in the Logistic influencing factor scale.A total of 300 patients with radiculopathy and patients with other types of cervical spondylosis were classified as 7: 3Logistic regression analysis was used to establish the prediction model.SPSS 27.0 and Stata17 data analysis software were used to verify and evaluate the differentiation and calibration degree of the model with ROC curve,Hosmer-Lemeshow goodness of fit test and calibration curve.The clinical validity of this model was evaluated by decision curve(DCA)analysis,and the prediction probability of this model was shown by Nomogram.In the second part,volunteers who met the inclusion criteria in the outpatient department of Acupuncture and Massage Rehabilitation Hospital of the First Affiliated Hospital of Yunnan University of Chinese Medicine were contacted through the clinical trial patient recruitment posters.A total of 68 patients with radicular cervical spondylosis were randomly divided into the stretching relaxation group and the conventional massage group,with 34 patients in each group.Based on the theory of meridian tendons,Chiropractic and Matiland were absorbed from traditional techniques,and the diseased muscles and soft tissues and joints were interfered with by "stretching","releasing" and "adjusting".Conventional massage group was treated with traditional massage techniques,3 times every 7 days,for a total of 2 weeks.Pain Degree(PD),Pain Rate(PR),Northwick park Neck Pain Scale,Zung Self-rating Anxiety Scale(SAS),and Zung Self-rating Depression Scale(SDS)were recorded before and after treatment and during follow-up.SPSS 27.0 software was used for data statistics and clinical efficacy analysis.Results:1.Sixty-eight patients were included,of which 65 had completed treatment,3 had abscission,33 were in the draft relaxation group and 32 were in the conventional massage group;There were no significant differences in gender,age,SAS and SDS between the two groups before treatment(P>0.05).2.There were differences in pain degree and pain rate between the two groups after treatment.Intra-group comparison showed that PD and PR values of the two groups decreased after treatment(P<0.05),while PD and PR values of the two groups showed no difference after treatment,and the curative effect of the two groups was stable(P>0.05).Comparison between groups: the effect of the tractive relaxation regulation group was better than that of the traditional massage group(P<0.001).3.There were differences in NPQ values between the two groups after treatment.Intra-group comparison showed that NPQ values of the two groups decreased after treatment(P<0.05),and NPQ values of the two groups were not different after treatment,and the curative effect of the two groups was stable(P>0.05).Comparison between groups: The effect of draft relaxation regulation group was better than that of traditional massage group4.There were differences in the SAS values between the two groups after treatment.Intra-group comparison showed that the SAS values of the two groups decreased after treatment(P<0.001),and there was no difference in the SAS values between the two groups after treatment,and the curative effect was stable(P>0.05).The stability of the therapeutic effect in the stretch Songtiao group was higher than that in the traditional massage group(P<0.001).Comparison between groups: The effect of draft relaxation regulation group was better than that of traditional massage group(P<0.05).5.The SDS values of the two groups were different after treatment,intra-group comparison: SDS values of the two groups decreased after treatment(P<0.05),SDS values of the follow-up were higher than before treatment,and the curative effect of the two groups was less stable(P<0.05);Comparison between groups: the effect of the tractor-loose modulation group was better than that of the traditional massage group(P<0.001),and the stability of the effect of the tractor-loose modulation group was higher than that of the traditional massage group(P<0.001).Conclusion:1.Logistic regression analysis showed that the physiological curvature of the cervical spine became straight,the duration of the disease was long,the cervical disc herniation,the constant exposure to cold and wet environment,and age were the main factors for the occurrence of nerve root symptoms and diseases.2.Stretch relaxation method can effectively relieve tension spasm of muscle soft tissue,reduce nerve compression symptoms,and relieve anxiety and depression of patients in psychological aspects.3.Logistic mathematical diagnosis model has a good predictive effect on the diagnosis of radiculopathy of cervical spondylopathy.It can be diagnosed clinically by identifying the above factors,avoiding related factors to improve the prevention and rehabilitation of the disease,and carrying out targeted treatment based on the mechanism of pathogenesis factors to improve the curative effect.
