Rotator cuff injury occurs when the rotator cuff and other tendon tissues are partially or completely damaged due to degenerative changes in the shoulder,trauma,insufficient blood supply and other factors.At present,the most effective treatment and repair method for rotator cuff injury is shoulder arthroscopic surgery.However,due to the special anatomy of rotator cuff and different rehabilitation treatment programs,the functional recovery of patients is not the same.In clinical practice,joint loosening and proprioceptive neuromuscular facilitation are the most popular rehabilitation methods for patients with rotator cuff injury after arthroscopy.Joint loosening techniques can effectively relieve pain and improve joint mobility restriction or stiffness.Proprioceptive neuromuscular facilitation can effectively improve muscle strength,motor control,balance and endurance direction.These two kinds of training can improve muscle strength,motor control,balance,endurance and range of motion,but they still have certain limitations in timeliness and technical optimization.Neuromuscular joint facilitation is a new type of exercise therapy.It integrates proprioceptive neuromuscular facilitation and joint loosening technique,and has the advantages of proprioceptive neuromuscular facilitation and joint loosening technique.It is different from a variety of previous treatment techniques: there will not be repeated treatment,the therapeutic effect has a quick effect and long-term.The neuromuscular joint facilitation was used in the treatment of patients with rotator cuff injury after arthroscopy,in order to achieve better therapeutic effect.Research objective: To investigate the efficacy of neuromuscular joint facilitation in patients with mid-to-late rotator cuff injury after arthroscopy,and to analyze its effect on pain,function,joint stability,and rotator cuff tendon retear rate.Research methods: From January 2022 to February 2023,60 patients admitted to the Rehabilitation Pain Department of Chongqing Ninth People’s Hospital after rotator cuff injury were randomly divided into three groups(neuromuscular joint facilitation group,joint loosening group and proprioceptive neuromuscular facilitation group),with 20 cases in each group.Patients in the three groups received routine rehabilitation training,joint loosening group was added to the routine rehabilitation training,proprioceptive neuromuscular facilitation group was added to the routine rehabilitation training,neuromuscular joint facilitation group was added to the routine rehabilitation training,20 minutes of routine training for each group,neuromuscular joint facilitation treatment,joint loosening and proprioceptive neuromuscular facilitation for 20 minutes each.The total time of exercise rehabilitation for each patient was 40 minutes,once a day,7 times a week,and all the three groups were treated for 2 weeks.The therapeutic effect was observed before,after the first week,after the second week,and 4 weeks after the end of treatment.Visual analog scale(VAS),joint motion Angle measurement in four directions of shoulder joint,Constant-Murley shoulder joint function score,joint position error test(flexion and abduction),clinical efficacy observation and MRI imaging examination of shoulder joint were respectively used,and finally statistical analysis was performed.Research results:1)Before treatment,the age,postoperative time and treatment observation indexes of the three groups were statistically analyzed and compared,and there was no significant difference(P > 0.05),which was comparable.2)After 1 week and 2 weeks of treatment,VAS score,shoulder range of motion,Constant-Murley shoulder function score and JPE test were significantly improved in three groups(P < 0.05).The effect of neuromuscular joint facilitation group on VAS score,shoulder range of motion,Constant-Murley shoulder function score and JPE test was significantly higher than that of the joint loosening group and PNF group(P < 0.05).However,in terms of Constant-Murley shoulder function score,there was no significant difference among the three groups in Constant-Murley shoulder function score 1 week after treatment(P > 0.05).3)In terms of clinical efficacy observation,after 2 weeks of treatment,the total effective rate of neuromuscular joint facilitation group(88.23%)> joint loosening group(55.56%)> proprioceptive neuromuscular facilitation group(55%),the difference was statistically significant(P < 0.05).4)In terms of rotator cuff retear observation,4 weeks after treatment,the retear rate of the neuromuscular joint facilitation group was 0 < joint loosening group(5.5%)and was lower than that of the proprioceptive neuromuscular facilitation group(6.25%),but there was no statistical significance(P > 0.05).Research conclusions:1)Neuromuscular joint facilitation is a safe and effective treatment method,which can effectively relieve the pain in the middle and late stage of arthroscopic rotator cuff injury and effectively improve the function of shoulder joint.2)Neuromuscular joint facilitation can effectively improve motor control and enhance proprioceptive function of patients after rotator cuff injury arthroscopy.3)Neuromuscular joint facilitation has no significant effect on rotator cuff retear prevention in patients with rotator cuff injury after arthroscopy.
