Explore Professor Nan Zheng’s Clinical Experience In Treating Diabetic Renal Failure Based On The Theory Of "State Target Discrimination"

Objective:Based on the theory of "state target discrimination",the clinical experience of Professor Nan Zheng in the treatment of diabetic renal failure was discussed,the theoretical system of diabetes renal failure was enriched,and the theoretical basis was provided for the inheritance of the experience of famous and old Chinese medicine,in order to guide clinical practice.Abide by the integrity of Chinese medicine,based on modulation,with the help of modern technology,let the target return to the state.Create the new medicine of medicine,combine the disease evidence,realize the use of the West for Chinese use,and promote the combination of China and the West.Methods: Collect and sort out the cases of diabetic renal failure personally diagnosed and treated by Professor Nan Zheng from January 2017 to November 2022,and summarize and sort out the prescriptions of Professor Nan Zheng for the treatment of diabetic renal failure.Excel is used to analyze basic information,prescription rules,symptom distribution and drug correlation using Excel,ancient and modern medical case cloud platform and SPSS Modeler.According to the results of data mining,combined with the research results of modern Chinese medicine pharmacology,under the supervision and guidance of the supervisor,the "state","target","cause" and "effect" of Professor Nan Zheng in the treatment of diabetic renal failure were analyzed.Results:1.A total of 247 patients who met the criteria were collected,including 167 males and 80 females.Of these,114 of the 167 male patients had a history of alcohol consumption;Eighteen of the 80 female patients had a history of alcohol consumption.There were 15 comorbidities,of which the top five were hypertension,hyperuricemia,azotemia,dyslipidemia,and coronary heart disease.2.Statistics on the course of 247 patients from the first diagnosis of thirst to the first diagnosis of diabetic renal failure,6-10 years of patients is the most.3.A total of 767 prescriptions were entered,involving 151 flavors of traditional Chinese medicine.There are 32 flavors with a frequency ≥ 400,and according to the data mining,the core prescription medicine hazel flower,lotus incense,zhuru,citrus fruit,rhubarb,oyster,American ginseng,ginger,purple river car,raspberry,quintuple seed,codonopsis,white cardamom,roshi vine,motherwort,tangerine peel,white zombie silkworm,cicada molt,white grass root,soil porridge,yellow essence,astragalus,danshen,magnolia,betel nut,grass fruit.4.Among the 767 cases in this study,563 cases were false state,511 cases were toxic,460 cases were stasis,and 454 cases were damaged state.A total of 110 clinical symptoms appeared,and the clinical symptoms of stasis were fatigue,frequent nocturia,thirst in the mouth,dry eyes,and chest tightness;The distribution of clinical symptoms of deficiency is more common,fatigue,frequent nocturia,shortness of breath,eyelid puffiness,and vomiting;The distribution of clinical symptoms of damage is more common as fatigue,frequent nocturia,foamy urine,lower limb edema,and mental fatigue;The distribution of toxic clinical symptoms is more common fatigue,mental tiredness,frequent nocturia,foamy urine,and vomiting.Conclusion: Professor Nan Zheng’s treatment of diabetic renal failure is divided into "three stages and four states",and a complete disease staging diagnosis and treatment system with "diabetes-diabetes nephropathy-diabetes renal failure" as the main line has been established,and the state of this disease is summarized as "stasis","deficiency","damage" and "toxicity" four states,and the stasis state is "cause",and the toxic state and damage state are the "fruit state" of the disease.The target of the disease is diabetes renal failure decoction.The target drugs for fatigue are purple river car,American ginseng,and astragalus;The target drugs for frequent nocturia are raspberries and goji berries;The target drugs for vomiting are Coptis and perilla leaves;The target drugs for lower limb edema are psyllium,poria and coix kernel;The target drugs for foaming urine are yellow essence and fivefold seed.The target drugs for urine protein were astragalus,huangjin,and quintuple;The target drugs of blood creatinine are poria and rhubarb.
