Clinical Study On The Treatment Of Early And Middle Stage Hyperthyroidism Based On The Theory Of "Fire Stagnation"

Objective: This study was based on the academic thought of "fire stagnation and development" theory in the treatment of hyperthyroidism(the heart and liver fire and exuberance syndrome)as the guidance,with the treatment method of dispersing Yu and dispersing fire,clearing heat and eliminating phlegm and gall,the patients were treated with oral decoction of traditional Chinese medicine,and the efficacy and safety of this method were objectively evaluated by observing and comparing the changes of clinical symptoms,efficacy indexes and safety indexes of the subjects before and after treatment.To provide more in-depth experimental data and new methods for clinical research of hyperthyroidism.Methods: In this study,72 patients who met the inclusion criteria of this study(including 20% increase to prevent shedding)were divided into 36 patients in the treatment group and 36 patients in the control group.The two groups were treated with basic treatment,the control group was treated with methimazole tablets,and the treatment group was treated with self-designed traditional Chinese medicine decoction on the basis of the control group.A total of 3 courses of treatment were observed.The differences of efficacy indexes such as thyroid function,thyroid stimulating hormone receptor antibody(TRAb),daily dosage of methimazole tablets,thyroid color ultrasound,TCM syndrome scores,and safety indexes such as blood routine,liver function,myocardial enzyme and urine routine before and after treatment between the two groups were observed and recorded.Results: Seventy-two patients were successfully included in this study.During the treatment,6 patients dropped out of the treatment group and the control group respectively,and 30 patients in the treatment group and the control group actually completed the treatment.After treatment,8 cases were effective,18 cases were effective and 4 cases were ineffective in the treatment group,with a total effective rate of 86.67%.In the control group,there were 4 cases of obvious effect,17 cases of effective,9 cases of ineffective,and the total effective rate was 70%.Treatment group was better than control group(P<0.05).Both groups were effective in improving thyroid function,TRAb,TCM syndrome scores,reducing the daily dosage of methimazole tablets,and reducing thyroid volume(P<0.05),but compared with the control group,the curative effect of the treatment group was significantly better than that of the control group(P<0.05).And the treatment group compared to the control group,the treatment group could prevent myocardial damage caused by hyperthyroidism,protect liver function and prevent leukopenia.In the course of treatment,the adverse reactions occurred less in the treatment group than in the control group,suggesting that the safety of the treatment group was higher.Conclusion: This study shows that treatment based on the theory of "fire stagnation and developmen" in the treatment of hyperthyroidism(heart and liver fire and exuberance syndrome),the dialectical thinking of syndrome should be adopted to dissipate fire and relieve heat.Compared with methimazole tablets alone,traditional Chinese medicine decoction combined with methimazole tablets can significantly improve the clinical symptoms of patients with traditional Chinese medicine,effectively regulate the thyroid function and TRAb level of patients,reduce the dosage of methimazole tablets,promote normal thyroid volume,protect liver function,prevent leukopenia,and prevent myocardial damage caused by hyperthyroidism.It has both efficacy and safety,and is worthy of further promotion and application.
