Objective: To explore the effect of family doctor contract management mode on blood glucose control in patients with type 2 prediabetes in the community,and to compare the awareness rate of prediabetes related knowledge and self-management ability before and after the intervention.To provide a management method for community doctors to manage patients with prediabetes.Methods: A total of 210 people diagnosed with prediabetes in a community of Anhui province from June 2021 to June 2022 were selected as the subjects of this study.Patients were randomly divided into observation group(management group)and control group by random method(random number table).The patients in the observation group(management group)established a contracted family doctor management model with the family doctor,which included the establishment of standardized health records,long-term health management into the community,the establishment of follow-up records,and health education lectures.The control group used the general community management model.The observation lasted for 6 months.The data were collected by questionnaire before and after the intervention,and the blood glucose control effect,the awareness rate of prediabetes knowledge and the change of self-management ability were observed and analyzed before and after the intervention.Results: After half a year of management,98 people in the observation group and 96 people in the control group completed the management.(1)Comparison of blood biochemical metabolism levels between patients in the management group and the control group included in the studyThere was no significant difference in blood biochemical metabolism levels(FPG,2h-PG,Hb A1c)between the two groups before the study management(P>0.05).The levels of blood biochemical metabolism(FPG,2h-PG,Hb A1c)in the observation group and the control group were significantly higher than those before the management,and the levels of blood biochemical metabolism(FPG,2h-PG,Hb A1c)in the observation group were significantly higher than those in the control group,the differences were significant(P<0.05).(2)Compare the BMI level of the management group and the control group before and after the study management.The results showed that there was no significant difference in body mass index(BMI)between the two groups before intervention(P>0.05);The body mass index(BMI)of the observation group after intervention was significantly lower than that before management,and the difference was statistically significant(P<0.05),the body mass index(BMI)of the control group was not significantly changed before and after the intervention,the difference was not statistically significant(P> 0.05).After intervention,the body mass index(BMI)of the observation group decreased more significantly than that of the control group,and the difference was statistically significant(P<0.05).(3)Comparison of the awareness rate of prediabetes knowledge between the two groups The results showed that there was no statistically significant difference between the two groups in the awareness rate of pre-diabetes before intervention(P>0.05);After intervention,the awareness rate of the two groups was significantly higher than that before intervention,and the difference was statistically significant(P<0.05);After management,the awareness rate of prediabetes in the observation group was significantly higher than that in the control group,and the difference was significant(P< 0.05)(4)comparison of self-management ability between the two groupsThe results of the study showed that before the management,the two groups of patients in the self-management ability level,There was no significant difference in the total score of self-management,health concept score,self-efficacy score,diet management score,exercise management score,rest and sleep score,stress and coping score,compliance management score,family environment management score,social environment management score(P > 0.05);And after the intervention,the scores of 10self-management statistics of patients in the two groups were significantly higher than those before the intervention,and the differences were statistically significant(P<0.05);Compared with the control group,the scores of 10 self-management statistics in the observation group increased more,and the differences were statistically significant(P<0.05).Conclusions: Through controlled experiments,this study found that the intervention of family doctor contract management mode can effectively reduce the blood glucose of patients with prediabetes,improve the cognitive level of patients with prediabetes,and promote the formation of good self-management ability of patients.This management mode is worthy of active promotion in the community and provides management methods for community general practitioners.To delay or reverse the progression of prediabetes to diabetes.
