| Objective: In this study,through the implementation of family doctor contract service management mode for hypertensive patients in the community,and the application of different scales to the patients before and after the management of blood pressure control,treatment compliance,quality of life and other objective evaluation,focus on the family doctor contract service management mode on hypertension patients in the community impact.In order to better improve the diagnosis and treatment of community hypertension patients and improve the quality of life,as well as to promote the prognosis of patients to provide an effective reference.Methods: Eighty patients with hypertension in Wan Zhi zhen Community Health Service Center from January 2021 to July 2021 were selected as the main subjects of this study,and divided two groups,which a study group(n=40)and a control group(n=40)according to the method of random number table.Under the condition of two groups of patients signed the informed consent,and the control group routine antihypertensive drug treatment and community intervention,the team take regular antihypertensive drug treatment combined signing the family doctor service health guidance and education,namely family service agreement signed with the patient,again by community nurses to the implementation of personalized and standardization management mode.Blood pressure control levels(diastolic blood pressure and systolic blood pressure)of the two groups were compared before and after intervention,and blood lipid levels such as cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C)and low density lipoprotein cholesterol(LDL-C)of the two groups were recorded for objective comparison.The level of self-management,treatment compliance and quality of life were also recorded.The database was established,and SPSS20.0 software package was used for statistical data processing.Statistical methods and analysis of variance were used to analyze the results of the data,and the changes and clinical significance of the above related indicators were analyzed.Results:(1)General information of the two groups of patientsA total of 80 patients with hypertension were included in this study,which were divided into the study group and the control group according to the method of random number table,with 40 cases in each group.The data of the patients in the study group included 26 males and 14 females,with an age of 50-62 years old,an average age of(58.12±8.47)years old,and a course of disease of 4-10 years.The mean duration of disease was(6.12±1.84)years;The control group consisted of 21 males and 19 females,with an average age of(59.40±6.82)years and a mean course of disease of(5.85±1.73)years from 52 to 65 years.After comparison,there was no statistical significance in age,and they were comparable(P>0.05).(2)Comparison of blood pressure control levels of patients in the study group and the control group After comparison,there was no statistical significance in the systolic blood pressure and diastolic blood pressure of patients in the two groups before management(P>0.05),but after management,the systolic blood pressure and diastolic blood pressure of patients in the two groups decreased significantly.The difference of blood pressure between the study group and the control group after management was statistically significant(P<0.05).(3)Comparison of blood lipid levels in the two groups before and after management,the differences of TC,TG,HDL-Cand LDL-C and other blood lipid levels between the two groups were statistically significant(P<0.05).The improvement level of the study group was more significant than the control group after management,the difference was statistically significant(P<0.05).(4)Comparison of self-management level of patients in the two groups after comparison,the diet management,medication management,emotion management,work and rest management,exercise management,condition monitoring and total score of patients in the study group were higher than those in the control group,the difference between the two groups was statistically significant(P<0.05).(5)Compared with the total compliance rate of the control group,the total compliance rate of the study group was higher,the difference between the two groups was statistically significant(P<0.05);(6)The quality of life of patients in the two groups was compared.The follow-up results showed that there was no statistical significance except for the physical pain index between the two groups(P>0.05);The total score of quality of life and the scores of related single indexes in the study group were higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion:The contrast experiments of the results of the study showed that the community hypertension patients the effect after signed by the family doctor service management is more precise,it is not only to a certain extent,control the patient’s blood pressure level,at the same time improve the patient’s treatment adherence and self-management ability,so as to further promote the prognosis of the patients quality of life,clinical treatment for patients provides the beneficial reference. |
