| 1.ObjectiveTo investigate the expression of Insulin-like growth factor-binding protein 7(IGFBP7)in plasma of patients with chronic heart failure.Meanwhile,the relationship between IGFBP7 and cardiac function,ventricular remodeling and inflammatory state in patients with chronic heart failure was analyzed to evaluate its clinical significance in chronic heart failure.2.MethodsA total of 116 patients with chronic heart failure who were hospitalized in the First Affiliated Hospital of Dali University from March 2022 to November 2022 were selected as the experimental group,and 48 patients with normal cardiac function who were hospitalized during the same period were selected as the control group.According to the classification criteria proposed by the New York Heart Association(NYHA)in the United States,patients with chronic heart failure were classified according to heart function.There are 39 patients with Grade II cardiac function,55 patients with Grade III cardiac function,and 20 patients with Grade IV cardiac function.According to the ejection fraction of cardiac ultrasound examination,59 cases were grouped with heart failure with preserved ejection fraction(HFp EF)group,21 cases were in the ejection fraction intermediate heart failure(HFmr EF)group,and heart failure with reduced ejection fraction(HFr EF)group of 36 patients.All selected patients underwent basic clinical features.Venous blood was collected on an empty stomach on the morning of the second day of admission for plasma IGFBP7,B-type natriuretic peptide,serum C-reactive protein,blood cell analysis,liver and kidney function,blood glucose,blood lipids and other blood biochemical analysis tests.Complete cardiac ultrasound in the first to three days of hospitalization.Apply statistical software SPSS25.0 to analyze and process data,The inspection criteria are(P<0.05).3.Results3.1 The plasma IGFBP7 concentration in the chronic heart failure group was higher than that in the control group,and the difference was statistically significant(P<0.05).3.2 Plasma IGFBP7 concentration was associated with cardiac function in patients with chronic heart failure,and plasma IGFBP7 concentration in patients with cardiac function grade IV was higher than that in patients with cardiac function grade II and III,and the difference was statistically significant(P < 0.05).The plasma IGFBP7 concentration was not statistically significant between HFp EF,HFmr EF and HFr EF groups(P>0.05).3.3 The plasma IGFBP7 concentration in the chronic heart failure group was associated with coronary heart disease,and the difference was statistically significant(P<0.05).3.4 Plasma IGFBP7 levels were positively correlated with left ventricular end-diastolic volume and left ventricular end-systolic volume,and negatively correlated with ejection fraction and right ventricular diameter.3.5 Plasma IGFBP7 levels were positively correlated with neutrophil value,lymphocyte ratio,serum C-reactive protein,and urea,and negatively correlated with albumin.4.ConclusionPlasma IGFBP7 is associated with cardiac function,cardiac structure and inflammatory status in patients with chronic heart failure,and can be used as one of the indicators to evaluate the condition of chronic heart failure. |
