| Objective : By comparing the plasma neurofilament light(Nf L)levels of patients with multiple sclerosis(MS)in acute phase,remission phase and control group,we explore the role of plasma Nf L in monitoring disease activity.By analyzing the correlation between plasma Nf L and MS patients’ age,disease duration,score of the expanded disability status scale(EDSS),number of MRI T2 phase and spinal cord lesions,and the number of MRI enhancement lesion,we investigate the role of plasma Nf L in monitoring disease severity and prognosis.Methods:(1)A total of 42 MS patients admitted to the outpatient department and inpatient Department of the Affiliated Hospital of Inner Mongolia Medical University from October2020 to February 2023 were collected,including 25 in acute stage and 17 in remission stage.All MS patients met the 2017 Mc Donald diagnostic criteria and they were classified as relapse-remission.(2)A healthy control group of 26 patients matching the age and gender of the case group was selected.The basic information of MS patients,such as age,gender,body mass index,course of disease was improved with the help of professional neurologists to improve the EDSS assessment to understand the severity of the disease,and the head,neck and chest plain scan and enhanced MRI examination was performed.(3)The plasma Nf L level of all patients and control group was detected by Simoa platform.Results:(1)The plasma Nf L level of MS patients in acute phase was higher than that in remission phase(P < 0.001).(2)The plasma Nf L level of MS patients in acute stage,remission stage and control group was compared,and the differences among the three groups were statistically significant.(3)There was no correlation between plasma Nf L level and age or course of disease(P=0.614;P=0.058),correlated with EDSS score,the number of MRI sub T2 and spinal cord lesions,and the number of MRI enhanced lesions(r=0.789,P < 0.001;r=0.846,P < 0.001;r=0431,P=0.005).Conclusion:(1)Plasma Nf L level is associated with activity,severity,and prognosis of multiple sclerosis.(2)Plasma Nf L has a good guiding effect on the diagnosis,course judgment and prognosis evaluation of multiple sclerosis,and is a valuable biological indicator with strong repeatability,which is expected to become a biomarker of multiple sclerosis.(3)The combination of plasma Nf L with clinical,MRI and other multi-mode evaluation is conducive to better monitoring of disease changes and optimization of treatment. |
