Research On The Academic Experience Of Traditional Chinese Medicine Master Wu Bingcai In Treating Postpartum Arthralgia Based On Data Mining

Objective:This study collects clinical cases of traditional Chinese medicine(TCM)master Wu Bingcai’s treatment of postpartum arthralgia,and applies modern data mining technology to explore the diagnosis and treatment rules of Professor Wu Bingcai’s treatment of postpartum arthralgia.The aim is to summarize the clinical characteristics of Professor Wu Bingcai’s treatment of postpartum arthralgia,hoping to inherit his valuable academic ideas,and provide reference for future clinical treatment of postpartum arthralgia.Methods:Clinical medical records of Professor Wu Bingcai’s treatment of postpartum arthralgia from January 1,2014 to November 30,2021 were collected,and the cases were screened according to diagnostic criteria and inclusion criteria.Excel software was used to input the patient’s general information,clinical symptoms,tongue images,pulse manifestation,prescriptions and other contents into the database.Using the cloud platform of ancient and modern medical records to create the network diagram of association rules of clinical symptoms;IBM SPSS Modeler 18.0 software was used to analyze association rules of Chinese medicine.And IBM SPSS Statistics 26 software was used to analyze factors of Chinese medicine.Results:In this study,a total of 340 prescriptions for treatment of postpartum arthralgia by Professor Wu Bingcai were collected,involving 90 patients.1.General information:(1)Among the 90 patients with postpartum arthralgia,the youngest was 21 years old,the oldest was 45 years old,and the most patients were between 26 and 30 years old.(2)The patient’s delivery included natural vaginal delivery,cesarean section delivery and medical abortion.(3)The solar terms with more patient visits are Cold Dew,White Dew,Minor Heat,etc.2.Four diagnostic information:(1)The tongue images of postpartum arthralgia patients are mainly divided into 5 types of tongue colors,5 types of tongue shapes,4types of tongue furs,3 types of coating colors,and 3 types of sublingual collaterals.The tongue colors are red tongue,light white tongue,light red tongue,etc;The tongue shape is more common with tooth-marked tongue,fat tongue,and cracked tongue;The fur color can be seen as yellow fur,white fur,and gray fur;The main types of tongue furs are greasy,thin,and thick;The sublingual collaterals are often swollen,blue and purple,as well as varicose.(2)There are 9 types of pulse manifestations,including stringy pulse,soft pulse,and weak pulse.(3)A total of 3,702 clinical symptoms of patients with postpartum arthralgia were integrated in this study.The common symptoms were chills,joint pain,sweating and aversion to wind.The network diagram of symptom association rules showed that there was a high correlation among chills,joint pains,sweating,dreaminess,fatigue and lack of strength.3.Formula statistics: Among the 340 prescriptions,Professor Wu Bingcai used a total of 35 formulas to treat postpartum arthralgia.Among them,Lishi Qingshu Yiqi Decoction was the most frequently used,followed by Ganmai Dazao Decoction,Fangji Huangqi Decoction,Chaihu Guizhi Decoction,Xiaoyao Powder,General Pain StasisExpelling Decoction,Shangzhongxia Tongfeng Decoction,Guipi Decoction,etc.4.(1)In this study,the cumulative frequency of use of Chinese medicine was 5945 times,involving 181 kinds of Chinese medicine.Among them,the drugs commonly used by Professor Wu Bingcai to treat postpartum arthralgia were baked licorice,Rhizoma Atractylodis Macrocephalae,Codonopsis pilosula,etc.(2)Efficacy statistical results showed that the most Chinese medicine has antiperspiration effect,followed by the effects of Chinese medicine such as miscarriage prevention,invigorating spleenstomach and replenishing qi,and inducing diuresis for removing edema.(3)The results of the integration of four qi and five flavors showed that the properties of Chinese medicine were mainly warm,mild and cold;The medicinal tastes are sweet,spicy and bitter.The scope of action mainly involves the spleen,lung,stomach meridian.(4)In the second-order drug association rules,the drug groups with high support were Ophiopogon japonicus-Codonopsis pilosula,Schisandra chinensis-Codonopsis pilosula,and Alisma orientalis-Codonopsis pilosula.Among the third-order drug association rules,the drug groups with high support were Ophiopogon japonicus,Codonopsis pilosula-baked licorice,Schisandra chinensis,Codonopsis pilosula-baked licorice,Alisma orientalis,Codonopsis pilosula-baked licorice,etc.;The network diagram of association rules of Chinese medicine suggested that baked licorice,Codonopsis pilosula,Schisandra chinensis,Astragalus mongholicus,Angelica sinensis,Ophiopogon japonicus,Pericarpium citri reticulatae and Cortex Phellodendri were the core drugs of Professor Wu Bingcai’s treatment of postpartum arthralgia,and they were highly correlated with each other.(5)Six common factors were extracted through factor analysis of Chinese medicine,namely F1: Rhizoma Atractylodis Macrocephalae,Codonopsis pilosula,Astragalus mongholicus,Angelica sinensis,Pericarpium citri reticulatae,Rhizoma Alismatis,Schisandra chinensis,Ophiopogon japonicus,Shenqu,the root of kudzu vine,Cortex Phellodendri,Radix Stephaniae Tetrandrae,immature tangerine peel,Rhizoma Atractylodis,and Rhizoma Cimicifugae;F2: Fructus Tritici Levis,baked licorice,and Fructus Jujubae;F3: Radix Bupleuri,Radix Paeoniae Alba,Radix Scutellariae,Ramulus Cinnamomi,and Rhizoma Pinelliae Preparatum;F4:parasite scurrula,Eucommia ulmoides;F5: Herba Asari,Radix Saposhnikoviae;F6:baked licorice.Conclusion:1.Etiology and pathogenesis: Professor Wu Bingcai believes that the postpartum arthralgia is the result of the combined action of internal and external causes.The external cause is due to being affected by pathogenic factors such as wind,cold,dampness,and heat.The internal cause can be summarized as five aspects: deficiency of ying and wei,deficiency of internal organs,improper diet,Emotional disorders,and imbalance of work and rest.The occurrence of diseases is nothing more than the“deficiency,depression,and stasis” caused by spleen dysfunction,liver dysfunction,and blood stasis obstruction.2.Syndrome Differentiation and Type Classification: Professor Wu Bingcai clinically divides postpartum arthralgia into three types: syndrome of disharmony between Ying and Wei,syndrome of rheumatic superficial deficiency,Damp heat stasis syndrome,syndrome of spleen deficiency with dampness,syndrome of liver depression and qi stagnation,and syndrome of stagnant blood obstructing the collaterals.Corresponding methods are proposed to expel wind and clear away cold,expel wind and remove dampness,clear heat and promote diuresis,invigorate spleen to eliminate dampness,soothe liver and regulate Qi,and activate blood circulation and remove blood stasis.These methods were used to eliminate pathogenic factors and support healthy energy,treat both manifestation and root cause of disease,and apply both smoothing and tonifying,so that the meridians can be unblocked and the arthralgia can be eliminated by itself.3.Treatment features: Professor Wu Bingcai follows the treatment principles of postpartum diseases in treating postpartum arthralgia,emphasizing “Do not be confined to the postpartum,and do not neglect after the postpartum”;Pay attention to the diagnosis and treatment of the patient’s throat in clinical diagnosis and treatment;For those with postpartum fever and dampness,medication should be carefully taken to prevent damage to the body;Professor Wu Bingcai advocates TCM classics and is deeply influenced by the theory of arthralgia syndrome in the Huangdi Neijing.He often starts from the theory of visceral arthralgia to differentiate and treat postpartum arthralgia;For the prevention and rehabilitation of postpartum arthralgia,Professor Wu Bingcai emphasizes the role of emotional care,dietary care,and daily living care.
