| Objective: This study mainly used the method of kidney,spleen,and blood drainage in patients with CKD3-5 who did not undergo dialysis,and to evaluate and analyze the improvement of TCM symptoms and related indicators before and after treatment.It provides certain ideas for the treatment of patients with CKD3-5(non-dialysis)with microinflammatory state and blood rheology in traditional Chinese medicine.Methods: A total of 92 patients with CKD3-5 who were hospitalized in the Department of Nephrology,Jiangxi Provincial Hospital of Traditional Chinese Medicine from 2019.6 to 2021.12 were enrolled,and46 patients were divided into 46 control groups and observation groups.The control group was treated as conventional medical advice,and the observation group was treated with the synergy of the prescription for kidney and spleen and blood drainage under the premise of the control group,and after a 3-month treatment cycle,the improvement of relevant indicators-basic vital signs,blood rheological indicators,TCM syndrome points,immune inflammation level,renal function,etc.was evaluated.Result:1.Basic data: The basic signs,CKD stage and course of the two groups were similar(P≥0.05),and there was a certain degree of comparability.2.TCM syndrome points: the clinical symptoms of TCM in both groups were reduced compared with before treatment,the total points of the observation group decreased from 15.78±2.32 points to 7.48±2.19 points,the control group decreased from 16.13±2.33 points to11.87±3.69 points,and the points of the two groups after treatment were compared,and the resulting T was 6.94 and P less than 0.05,which confirmed that the observation group used traditional Chinese medicine and conventional medical advice to cooperate with diagnosis and treatment with excellent results.3.Clinical total effective rate: the total effective rate of the observation group after treatment was 82.61%,which was better than that of the control group(52.17%),and the test showed that X2=9.96,P≤0.01.4.Improvement of renal function: comparing BUN,Scr,β2-MG,GFR,and 24 h urine albumin in the two groups,and the P≤0.05 was compared between the groups,which was statistically significant,and the observation group was obviously dominant.5.Changes in inflammatory indexes: comparing the relevant indexes of the two groups,after treatment,the CD4+,IL-10 and CD8+ levels of patients in the two groups were adjusted upward compared with the previous period,TNF-α,CRP and IL-6 indexes were lower than before treatment,and the P ≤ 0.01 between the groups was statistically significant,and the treatment effect of the observation group was significantly higher than that of the control group.6.BRE index: compare the plasma viscosity,high-cut whole blood viscosity,low-cut whole blood viscosity and FIB related indexes of the two groups.The blood rheological level of the two groups after treatment was adjusted accordingly,and the P≤0.01 was statistically significant between the groups,and the treatment effect in the observation group was significantly better than that in the control group.7.In terms of safety and adverse reactions,there were no side reactions in both groups.Conclusion: The method of promoting the kidney and spleen and activating blood turbidity can significantly improve the clinical manifestations of traditional Chinese medicine and glomerular filtration rate,anti-inflammatory,improve blood rheology,reduce urine protein,delay kidney damage in patients with CKD3-5 stage nondialysis,and no adverse reactions were found in clinical experiments,which clearly indicates that traditional Chinese medicine can effectively treat the microinflammatory state and blood rheological changes of CKD patients,which can be further studied and applied clinically. |
