Objective:By comparing the number and density of mast cells in the gastric mucosa,as well as the degree of degranulation and psycho-psychological differences between patients in the bile reflux group and those in the non-bile reflux group,we initially investigated the mechanism of bile reflux at the macroscopic and microscopic levels,so as to provide reference for the treatment and prevention of bile reflux patients.Methods:Patients who met the exclusion criteria and inclusion criteria were selected from our gastroenterology outpatients and inpatients,and those who met the criteria were included in the bile reflux group or non-bile reflux group with reference to Kellosalo classification,which minimizes false-positive bile reflux caused by nausea reaction due to gastroscopy insertion.Bile reflux was classified into degree I,II,and III according to the amount and color(clear,mild yellow-green,and moderate to deep yellow-green)of the liquid in the mucus lake under gastroscopy,and those without bile reflux were included in the non-bile reflux group,with a total of 60 cases in both groups.Also,patients in both groups met the exclusion criteria,i.e.,no gastrectomy and no gallbladder disease,diabetes mellitus,hepatitis,pancreatitis,or other diseases affecting this study.The patients in both groups were treated as follows:(1)Biopsy forceps were performed under gastroscopy on the body or sinus of the gastric mucosa in both groups,and the biopsy specimens were fixed in paraffin,sectioned,and specially stained with toluidine blue.After tissue processing,five fields of view were randomly selected under a light microscope at 400× to count the mast cell count and percentage of mast cell degranulation(percentage of mast cell degranulation= number of mast cell degranulation/total mast cell count*100%),and the statistical processing results of the two groups were used to elaborate whether there was a correlation between gastric mucosal mast cells and bile reflux in the two groups;(2)the above enrolled patients were treated with HAD scale and GSRS scale were used for scale investigation,and the data were collected and statistically analyzed whether there was correlation between anxiety and depression symptoms and bile reflux;(3)the basic data of patients’ age,gender,degree of atrophy,degree of erosion,HP infection,etc.were collected and statistically analyzed for correlation with bile reflux.Results:(1)Mast cell count: After toluidine blue special staining of the pathological sections of gastric mucosa,five randomly selected 400× microscopic fields were used to count mast cells,and it was found that the mast cell count was higher in the former group compared with the non-bile reflux group,and the difference was statistically significant(P < 0.01);(2)Percentage of mast cell degranulation: mast cells were observed under a light microscope at 400× Morphology: normal mast cells had a round and large morphology with intact cell membranes,and the granules appeared purplish red or red.In the case of degranulated mast cells,the morphology was changed,the cell membrane was ruptured,the granules overflowed outside the cell membrane,and sometimes vacuoles or uneven granule coloring were seen.The percentage of mast cell degranulation(percentage of mast cell degranulation = number of mast cell degranulation/total mast cell count*100%)was counted,and it was found that the percentage of mast cell degranulation was higher in the bile reflux group compared with the non-bile reflux group,and the difference was statistically significant(P < 0.01).(3)Statistical analysis was performed on the age,gender,degree of atrophy,degree of erosion,and HP infection in both groups,and the differences were not statistically significant(P > 0.05).Conclusion:The occurrence of bile reflux was significantly correlated with the number of mast cells and the degree of degranulation of gastric mucosa(P < 0.01),but there was no correlation with psychological factors,age,gender,degree of atrophy,degree of erosion,or HP infection(P > 0.05);the mechanism of bile reflux may be closely related to the proliferation of gastric mucosal mast cells and their degranulation and release of various substances,but the action The mechanism of bile reflux may be closely related to the proliferation of gastric mucosal mast cells and their degranulation and the release of various substances,but the specific action of the targets still needs further investigation.
