Objective:The purpose of this study was to explore the feasibility of using the relative content of serum miR-124 、 miR-132 、 miR-134 as a diagnostic basis for early screening of post-stroke cognitive impairment(post-stroke cognitive impairment,PSCI).In order to study whether the relative content of serum miR-124,miR-132,miR-134 can be used as an objective index for early diagnosis of PSCI and early intervention in patients with stroke.Methods:Collect clinical data of 79 first-episode stroke patients who were hospitalized in the Department of Neurology at Linyi People’s Hospital from May 2021 to May 2022.On the second day of admission,collect patient serum samples and store them in a-80 ℃refrigerator.At the same time,clinical data of 36 healthy control groups matched in gender,age,and education level were collected at the physical examination center as age matched controls(AMC).Serum samples were collected and stored in a-80 ℃refrigerator.After 6 months,follow-up visits were conducted on the 79 stroke patients mentioned above.On the day of follow-up,the same rehabilitation therapist used the Mini Mental State Examination(MMSE)to measure the cognitive function level of the patients.Excluding 12 individuals who were lost to follow-up,they were divided into the PSCI group(n=33)and the post stroke cognitive normal(PSCN)group(n=34)based on whether they had post stroke cognitive impairment.The target miRNAs in three groups of serum were extracted in the laboratory,and the expression levels of miR-124,miR-132,and miR-134 in serum were detected by real-time quantitative polymerase chain reaction(q RT PCR).The relative expression of miRNAs in serum was calculated by logarithmic transformation of 2-△Ctmethod(Log102-△Ct).To explore the relationship between serum miR-124,miR-132,and miR-134 and post stroke cognitive impairment,univariate analysis of variance was used to compare the relative expression levels of miR-124,miR-132,and miR-134 among three baseline data sets,as well as the Mini Mental State Examination(MMSE)score.Pearson and partial correlation analysis were also used to analyze the correlation between miR-124,miR-132,miR-134,and MMSE score.To further investigate the predictive value of miR-124,miR-132,and miR-134 on PSCI,the receiver operating characteristic curve(ROC)was used to analyze the predictive value of miR-124,miR-132,and miR-134 on PSCN.Apply logistic binary regression fitting analysis to screen independent influencing factors for each indicator.Results:Among the 67 stroke patients collected in this study,34 developed PSCI,with a disease incidence rate of 50.74%.Compared with the other two groups,the relative expression of miR-124,miR-132,and miR-134 in the serum of PSCI patients was significantly upregulated by q RT-PCR,and the difference was statistically significant(p<0.001).Pearson and partial correlation analysis showed that the MMSE score of the PSCI group was lower than that of the PSCN and AMC groups,with a statistically significant difference(p<0.05).The relative expression levels of serum miR-124,miR-132,and miR-134 were strongly negatively correlated with the MMSE score(p<0.05);Further establishing the ROC curve and calculating the AUC(95% CI),it was found that the AUC predicted by miR-124,miR-132,and miR-134 for PSCI were 0.851,0.895,and 0.831,respectively(p<0.001).Among them,the relative expression level sensitivity of serum miR-124 was 75.8%,and the specificity was 91.2%;The sensitivity and specificity of the relative expression level of miR-132 in serum were 81.8% and82.4%,respectively;The sensitivity and specificity of the relative expression level of miR-134 in serum were 72.7% and 88.2%,respectively.Applying logistic binary regression fitting analysis to screen independent influencing factors for each indicator,it was found that the AUC of miR-124+miR-132+miR-134 predicting PSCI was 0.902,which was greater than that predicted by a single indicator(p<0.001).Conclusions:1.MiR-124,miR-132 and miR-134 were significantly upregulated in the serum of patients with cognitive impairment after stroke.2.The higher the expression level of miR-124,miR-132 and miR-134 in serum,the greater the possibility of PSCI.It is helpful for clinical early identification of cognitive impairment after stroke. |
