| Objective:1.To assess the impact of the intervention techniques based on management by objectives theory on the incidence of chemical cystitis and knowledge of bladder infusion chemotherapy in patients undergoing intravesical infusion chemotherapy2.To assess the impact of the intervention techniques based on management by objectives theory on anxiousness,depressed mood,and life quality for patients receiving intravesical infusion chemotherapy.3.To assess the impact of the intervention techniques based on management by objectives theory on the drug spillage rate and the implementation of intravesical chemotherapy occupational safety measures during intravesical infusion chemotherapy.Methods:1.The convenience sample approach was used to identify patients who satisfied the inclusion and exclusion criteria and were being treated for the first time with intravesical infusion chemotherapy in the Urology Outpatient Department of a tertiary hospital in Taiyuan City,Shanxi Province.Using a non-contemporaneous controlled experimental research method,the patients from May to August 2022 were the control group,and the patients from September to December 2022 were the intervention group,with 66 people in each group.2.Interventions were performed during the induction infusion chemotherapy phase of the patients,which lasted for a total of 2 months.The control group received routine bladder perfusion measures and routine health education,and the intervention group received interventions based on management by objectives theory on the basis of the control group:First set up a target management team and established a target management list,which includes three aspects: patients,medical staff and the environment.Clarify the goals and intervention methods that individuals need to achieve at each level and form an intervention plan for target management theory.Then each goal executor completed their sub-goals one by one,including improving the department’s bladder perfusion chemotherapy environment,optimizing and standardizing some bladder perfusion procedures,training and supervising bladder perfusion operators,and strengthening patient health education.The team leader supervised regular inspections.3.Data on intravesical infusion chemotherapy knowledge,incidence of chemical cystitis,anxiety and depression,and quality of life were obtained before,1 month,and 2months after the intervention.The drug spillage rate and the implementation of occupational protective measures of intravesical chemotherapy were collected when the medical staff performed intravesical infusion chemotherapy on the two groups of patients.Statistical analysis was performed using SPSS 22.0 to judge the intervention effect.Results:1.Two patients withdrew from the trial during the intervention period,and a total of130 patients completed the study,including 66 cases in the control group and 64 cases in the intervention group.There was no significant differences between the two groups of patients in terms of gender,age,marital status,location of residence,employment,current working status,education level,medical payment status,medical payment method,tumor size,number of tumors,or illness course(P<0.05).2.Following 1 and 2 months of intervention,there was no significant difference in the frequency,urgency,hematuria and dysuria symptoms between the two groups(P<0.05);the incidence of urethral pain symptoms was considerably lower in the intervention group than in the control group(P<0.05),and the intravesical infusion chemotherapy knowledge scores was significantly higher in the intervention group than in the control group(P<0.05).3.Prior to the intervention,the scores of anxiety and depression in the two groups were at moderate level.The anxiety and depression scores of the intervention group were substantially lower than those of the control group after 1 and 2 months of intervention(P<0.05).Anxiety and depression were severe in the control group after 2 months of intervention,but moderate in the intervention group.4.Prior to the intervention,the intervention group included physical,emotional,role,cognitive and social functioning,global functioning and nausea/vomiting were considerably lower than the reference values(P<0.05),whereas sleepless was significantly greater(P<0.05).1 and 2 months after the intervention,the intervention group’s physical,emotional,role,cognitive and social functioning,global functioning scores were significantly greater than the control group’s(P<0.05),while pain,urinary symptom,intravesical treatment problem,sleepless and future perspective were significantly lower than the control group’s(P<0.05).Following 2 months of intervention,the intervention group’s physical,role,emotion,cognitive and social functions,pain,and sleepless were considerably greater than the reference values(P<0.05).5.After to the intervention,the drug spillage rate in the intervention group was significantly reduced(P<0.05).After 2 months of intervention,the drug spillage rate in the intervention group was significantly lower than that in the control group(P<0.05).6.Prior to the intervention,the qualifying rate of random inspection in the control group was 62.34%,whereas it was 55.56% in the intervention group.Following the intervention,the implementation rate of adequate preparation of protective items before operation,standard wearing,correct handling of drug spillage during drug injection or extubation and strict waste disposal was significantly greater in the intervention group compared to the control group(P<0.05).Following 2 months of intervention,the qualified rate of random inspection in the control group was 61.04%,whereas it was 88.89% in the intervention.Conclusion:The intervention strategy based on management by objectives theory can significantly reduce the incidence of urethral pain in patients with chemical cystitis,effectively improve the level of anxiety and depression in patients,effectively improve the knowledge level of intravesical infusion chemotherapy and quality of life in patients,significantly reduce the drug spillage rate of medical staff during bladder perfusion operation and effectively improve the implementation rate of occupational protection measures for intravesical infusion chemotherapy,can fundamentally improve the quality of diagnosis and care,and thus ensure the safety of medical staff and patients,it is worthy of clinical promotion application. |
