| Objective:The radiomics model based on full-field digital mammography(FFDM)of intratumoral,peri-tumoral and contralateral breast parenchyma was used to identify benign and malignant BI-RADS 4 breast lesions to providing radiologists with an effective adjunctive opinion,thereby minimizing unnecessary biopsies in patients,helping to improve the early detection rate of breast cancer and improving patient prognosis.Methods:The patients of BI-RADS 4 sessions who underwent FFDM examination at our hospital from November 2020 to August 2021 were retrospectively analyzed,and 117 patients were included according to the Inclusion and discharge criteria.The patients were divided into two groups according to pathological criteria: benign lesions and malignant lesions,and the images were annotated with ROI and extracted and downscaled with radiomics features using relevant software,and the screened features were analyzed,and single-region radiomics models and multi-region radiomics models were constructed,and then clinical models and joint clinical-radiomics models were constructed by combining the imaging features of patients and common clinical data,and finally,the The diagnostic utility of different models for the BI-RADS 4 lesions and whether there were differences between different models were evaluated.Results:Among the 117 subjects included in the study,there were 51 benign cases and 66 malignant cases.Single-region radiomics models were constructed in each of the 3different regional configurations.Except for the contralateral parenchymal model,all of them could effectively identify the nature of the BI-RADS 4 lesions.The combined model was superior to both the single-region radiomics model and the clinical model,and was superior to the radiologist.The AUC of the intratumoral,peritumoral,contralateral parenchymal,clinical,and combined clinical-radiomics models in the test set were 0.80,0.74,0.52,0.79,and 0.87,respectively,with sensitivities of 0.74,0.74,0.03,0.80,and 0.88,and specificities of 0.88,0.78,0.98,0.77,and 0.87,respectively.Conclusion:The FFDM-based intratumoral and peri-tumoral radiomics models can identify the nature of BI-RADS 4 lesions in the breast,and the combined model can further improve the diagnostic efficacy of BI-RADS category 4 lesions,but the contralateral parenchymal texture model cannot identify the benignity or malignancy of breast lesions. |
