Objective:To validate the Family Nurses Diet Theory proposed by our team, under the guidance of this theory, this study combined with the characteristic agricultural product tartary buckwheat in Shanxi region to construct a dietary therapy plan suitable for Shanxi residents, and verified and evaluated the dietary therapy plan, providing scientific basis and support for the dietary care of patients with fatty liver.method:1)Family Nurse Diet plan: under the guidance of the "Family Nurse Diet Theory",using literature analysis by literature research and analysis, the first draft of the family nurse diet plan was constructed through group meetings.Relevant experts were invited to hold an expert meeting to put forward targeted opinions and suggestions on the first draft of the family nurse diet plan, and the final draft of the family nurse diet plan was determined through a group meeting according to experts’ opinions.2)Randomized controlled trials: evaluating the feasibility and effectiveness of the family nurse diet plan. From October 31,2022 to November 17,2022,62 patients with metabolic dysfunction-associated fatty liver were recruited from a community health service center in Shanxi Province. All subjects were randomly divided into a tea group and a diet group by using the random number table method, and there were 31 fatty liver subjects in each of the two groups. The tea group brewed 2 packs of Tartary buckwheat tea every day and were given routine health education. The diet group brewed 2 packs of Tartary buckwheat tea every day and were given the family nurse diet plan. The two groups were intervened for 42 days, and the outcome indicators were observed, measured, analyzed and evaluated before and after the intervention.Result:1)Construction of Family Nurse Diet therapy plan: Under the guidance of "Family Nurses Diet therapy Theory",the family nurse diet intervention program was constructed using literature analysis and expert meeting methods. The program has 8 first-level categories and 10 second-level categories, and it is constructed from eight aspects:nutritional status assessment, dietary survey, lifestyle survey, comorbidity survey, personalized recipe formulation, behavior counseling,follow-up,adverse reactions and treatment.2)Dietary status: After short-term application of this dietary regimen in MAFLD patients, there were significant changes in the dietary structure and food selection of the dietary therapy group. The proportion of total energy intake to fat intake decreased, while the proportion of protein intake increased(P<0.05).The only fat intake in the tea drinking group decreased significantly(P<0.05).Compared with the two groups, the total energy intake of the dietary therapy group was significantly reduced Both protein and fat showed significant improvement compared to the tea drinking group(P<0.05).At the same time, the intake of pork and refined grains in the dietary therapy group significantly decreased compared to before(P<0.05),while the intake of non starch vegetables, chicken, and coarse grains increased significantly compared to before(P<0.05).The intake of non starch vegetables, refined grains, and coarse grains in the tea drinking group only improved after42 days(P<0.05).Compared to the two groups, the intake of pork and refined grains in the diet group was significantly lower than that in the tea drinking group(P<0.05),while chicken and coarse grains were higher than those in the tea drinking group(P<0.05).3)Fatty liver relief status: In the dietary therapy group, there were a total of 15 subjects with improved fatty liver grading, with a relief rate of 48%.In the tea drinking group, there were 3 subjects with a relief rate of 10%,which was statistically significant(P<0.05)compared to the two groups. The relief rate was 0% for subjects with a dietary compliance score of 11-21,40% for subjects with a score of 22-32,and 72.2% for subjects with a score of 33-44.4)Anthropometry indicators: the improvement of anthropometry indicators of subjects in the tea drinking group was not significant(P>0.05),while the weight(W),waist circumference(WC),hip circumference(HC),and body mass index(BMI)of subjects in the diet therapy group were significantly improved(P<0.05).Compared with the two groups, there was a significant decrease in weight, waist circumference, and hip circumference(P<0.05).5)Glucose and lipid metabolism: the improvement of glucose and lipid metabolism indexes of the subjects in the tea drinking group was not obvious(P>0.05).After 42 days of intervention, the subjects in the dietotherapy group significantly decreased fasting blood glucose(FBG),total cholesterol(TC),total triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)(P<0.05).Compared with the two groups,FBG,TG After intervention,LDL-C significantly improved(P<0.05),confirming that this protocol may have a certain effect on improving metabolic disorders.6)Liver and kidney function: The aspartate aminotransferase(AST)and uric acid(UA)levels in the tea drinking group decreased, while the alanine aminotransferase(ALT)and AST levels in the dietary therapy group did not show significant differences compared to the tea drinking group, but were significantly reduced before and after intervention(P<0.05).There were no significant changes in creatinine(SCr)and urea nitrogen(BUN)in the dietary therapy group(P>0.05),indicating that short-term application of this regimen effectively improves liver enzymes in MAFLD patients, reduces liver damage, and does not cause damage to renal function. Conclusion:In order to verify the model of "Family Nurse Diet Theory",the study constructed a family nurse diet intervention program. After 42 days of application,it is confirmed that the program is feasible and effective, and MAFLD patients benefit from this diet program;...
