Objective: Constructing a nasal tamponade care intervention programme for patients undergoing nasal endoscopic surgery for chronic sinusitis.To validate the effectiveness of an evidence-based care programme for nasal stuffing in patients undergoing nasal endoscopic surgery for chronic rhinosinusitis.Methods:(1)Construction of a nasal tamponade care protocol for patients undergoing nasal endoscopic surgery for chronic rhinosinusitis: we systematically searched the literature and guidelines related to nasal stuffing care for patients undergoing nasal endoscopic surgery for chronic sinusitis,screened the literature,extracted and summarized the relevant evidence,evaluated the quality of the evidence,and finally obtained the best research evidence.Members of the group discussed,organized and summarized to form the first draft of the nasal stuffing care plan for patients undergoing nasal endoscopic surgery for chronic rhinosinusitis.Experts in the field of otorhinolaryngology-head and neck surgery were invited to evaluate the extracted evidence using expert meeting method and to propose modifications to revise and improve the preliminary draft of the nasal stuffing care protocol.(2)Effectiveness evaluation of nasal tamponade care program in patients undergoing nasal endoscopic surgery for chronic rhinosinusitis: a total of 68 patients with chronic rhinosinusitis undergoing nasal endoscopic surgery who met the nadir criteria were conveniently selected in a tertiary care hospital in Gansu Province using the quasi-experimental study,with patients included from February to May 2022 as the control group and patients included from June to October 2022 as the intervention group.The control group received routine perioperative care for nasal endoscopic surgery,and the intervention group received a constructed nasal tamponade care protocol along with routine perioperative care.The General Information Questionnaire,Visual Analogue Scoring(VAS)scale and Nasal Sinus Outcome Measure 22(SNOT-22)scale were used as evaluation tools to evaluate patients before,one week after,and one month after surgery,respectively.The data were statistically analyzed using t-test,X2test and repeated measures ANOVA to compare the quality of survival between the two groups and to validate the effectiveness of the nasal cavity tamponade care protocol.Results:(1)A total of 18 papers were included in this study after literature search and screening,including 4 guidelines,2 expert consensuses,1 quasi-experimental study,2systematic reviews,and 7 RCTs.The care protocol including 16 best evidence was initially constructed through evidence extraction and quality evaluation.A total of 12 experts participated in the expert meeting to revise and delete the protocol.The expert authority coefficient Cr = 0.89(greater than 0.8),with a high degree of expert authority and reliability of the protocol.The result was a nasal tamponade care protocol for patients undergoing nasal endoscopic surgery for chronic sinusitis.(2)A total of 68 patients undergoing nasal endoscopic surgery for chronic rhinosinusitis were included in the study,34 cases in the intervention group and 34 cases in the control group,with no cases of loss of visit.(1)The general data(gender,age,education level,history of nasal surgery,etc.)of the two groups were compared,and the differences were not statistically significant(P>0.05),and the baseline data of the two groups were comparable.(2)Repeated measures ANOVA showed that the differences in VAS and SNOT-22 scores between the two groups were statistically significant(P < 0.05)at different time points,and the scores tended to decrease over time;the differences were statistically significant(P < 0.05)when the VAS and SNOT-22 scores of the two groups were compared at all time points;at the time point and group interaction,the two groups of patients Both VAS and SNOT-22 scores were statistically significant(p < 0.05).The results showed that the VAS and SNOT-22 scores differed between the two groups,with the intervention group showing a higher decrease than the control group.Conclusions: This study constructed a systematic nasal tamponade care protocol for nasal endoscopic surgery for chronic sinusitis.The nasal tamponade care protocol constructed in this study for patients undergoing nasal endoscopic surgery for chronic rhinosinusitis was able to improve patient comfort and enhance the quality of patient survival.
