Clinical Study Of Song’s Lijin Bone-setting Technique Combined With Xiaoding Ointment In The Treatment Of Chronic Ankle Joint Instability

Objextive : To observe the clinical effect of Song’s Lijin bone-setting technique combined with external application of Xiaoding ointment in the treatment of CAI,and to provide reference for clinicians in the conservative treatment of CAI.At the same time,through this clinical study,the action mechanism of Song’s Lijin bone-setting technique combined with external application of Xiaoding ointment in the treatment of CAI is discussed and summarized,and the operation process of Song’s Lijin bone-setting technique in the treatment of CAI is standardized,so as to facilitate clinical application and promotion,and further inherit and carry forward the academic thoughts of Professor Song Guijie.Methods:Sixty CAI patients with unilateral ankle instability admitted to the orthopedics Department of the Affiliated Hospital of Gansu University of Chinese Medicine from January2022 to December 2022 were selected and divided into treatment group and control group according to random number table method,with 30 cases in each group.In the treatment group,there were 16 males and 14 females,aged 18～53(30.40±7.97)years,the shortest course of disease was 9 months,the oldest 24 months;In the control group,there were 13 males and 17 females,aged 19～47(32.27±8.30)years old.The shortest course of disease was8 months and the oldest 21 months.The treatment group was treated with Song’s Lijin bone-setting technique combined with Xiaoding ointment external application,and the control group was treated with Xiaoding ointment external application,the course of treatment was 4weeks.VAS scores and AOFAS functional scores were recorded before,2 weeks and 4 weeks after treatment in 2 groups,as well as Cumberland ankle instability assessment questionnaire scores,length of standing on one foot with eyes closed,swelling degree,and ultrasonic measurements of ankle anterior talofibular ligament thickness and calcaneal fibular ligament thickness in 2 groups before and 4 weeks after treatment,respectively.SPSS25.0 statistical software was used to analyze the data of the two groups,and to evaluate the clinical efficacy of Song’s Lijin bone-setting technique combined with external application of Xiaoding ointment in the treatment of CAI.Results:1 There were no significant differences in baseline data and observation indicators before treatment between the two groups(P > 0.05),indicating comparability between the two groups.2 VAS score: Comparison of VAS scores before and after treatment between the two groups showed statistically significant differences(P < 0.05),indicating that both treatments were effective in improving ankle pain in CAI patients.After treatment,the difference between the two groups was statistically significant(P < 0.05),indicating that Song’s Lijin bone-setting technique combined with Xiaoding ointment external application was better than Xiaoding ointment external application in improving pain in CAI patients.3 AOFAS score: Intra-group comparison of AOFAS scores before and after treatment between the two groups showed statistically significant differences(P < 0.05),indicating that both treatments were effective in improving ankle joint function in CAI patients.After treatment,the difference between the two groups was statistically significant(P < 0.05),indicating that Song’s Lijin bone-setting technique combined with Xiaoding ointment external application was better than Xiaoding ointment external application in improving the function of CAI patients.4 CAIT score: Intra-group comparison of CAIT score before and after treatment between the two groups showed statistically significant differences(P < 0.05),indicating that both treatments could help improve the ankle stability score of CAI patients.After treatment,the difference between the two groups was statistically significant(P < 0.05),indicating that Song’s Lijin bone-setting technique combined with Xiaoding ointment external application had better effect than Xiaoding ointment external application in improving the ankle stability score of CAI patients.5 Static balance ability: Intra-group comparison of standing time before and after treatment showed statistically significant difference(P < 0.05);There was no significant difference in standing time before and after treatment in the control group(P > 0.05).After treatment,the difference between the two groups was statistically significant(P < 0.05).The results showed that in terms of improving the static balance ability of CAI patients,Song’s Lijin bone-setting technique combined with Xiaoding ointment external application had obvious effect,while Xiaoding ointment external application had no obvious effect.6 Degree of ankle swelling: The degree of ankle swelling after treatment in both groups was significantly improved compared with that before treatment,with statistical significance(P < 0.01),indicating that both treatments were effective in improving the degree of ankle swelling in CAI patients.After treatment,there was no significant difference in the degree of swelling between the two groups(P > 0.05),indicating that the two treatment schemes had similar efficacy in detumescence,but there was no difference.7 Ultrasonic measurement of lateral collateral ligament thickness of ankle joint: After treatment,the thickness of anterior talofibular ligament and calcaneal ligament in both groups was significantly improved compared with that before treatment,with statistical significance(P < 0.05),indicating that both treatments were effective in improving the thickness of ligaments in CAI patients.After treatment,there were statistically significant differences in the thickness of anterior talofibular ligament and calcaneal ligament between the two groups(P < 0.05),indicating that manipulation combined with external application of Xiaoding ointment was better than external application of Xiaoding ointment in improving ligament thickness.8 Overall efficacy: healing rate of treatment group: 83.3%,total effective rate: 96.7%;The apparent rate of the control group was 46.6%,and the total effective rate was 86.7%.The apparent rate and total effective rate of the treatment group were higher than those of the control group.9 Assessment of adverse reactions: No adverse events occurred in both groups during treatment.Conclusion: Song’s Lijin Bone-fixing manipulation combined with Xiaoding ointment external application can effectively reduce ankle pain and swelling in CAI patients,improve ankle joint function and the thickness of the anterior talofibular ligament and calcaneal fibular ligament,and improve ankle joint stability and static balance ability,which is worthy of clinical application.
