Objective: Crohn’s disease and ulcerative colitis are chronic non-specific intestinal inflammatory diseases that etiology is not yet clear.inflammatory bowel disease has the characteristics of prolonged,recurrent attacks and remission,and patients with Crohn’s disease are often combined with anemia,fatigue and other systemic manifestations,which seriously affect the quality of life of patients.At present,a few literature reports suggest that vitamin D may be associated with anemia in patients with Crohn’s disease.The purpose of this study is to explore the relationship between serum vitamin levels and anemia in patients with Crohn’s disease.Methods: A total of 40 patients hospitalized in the Third Affiliated Hospital of Guangzhou Medical University from 2022-07-01 to 2023-03-31 and diagnosed with Crohn’s disease were included,including 27 cases in remission stage(CDAI<150)and 13 cases in the active stage(CDAI≥150),and the diagnostic criteria refer to the "Consensus Opinion on the Diagnosis and Treatment of Inflammatory Bowel Disease(Beijing,2018)".Measure and collect hemoglobin and serum 25(OH).D The relationship between vitamin D and hemoglobin and other indicators was compared and analyzed.Results:30% and 90% of patients with Crohn’s disease have anemia and vitamin D insufficiency,respectively;Hemoglobin levels in moderately and mildly active patients were lower than those in remission(median 112.00 vs 135.00 vs 145.00,P<0.01);vitamin D levels in moderately and mildly active patients were lower than patients in remission(mean 13.884 vs 19.047 vs 21.810,P<0.02);hemoglobin levels decreased with decreasing vitamin D levels in patients with Crohn’s disease [normal vitamin D levels(median 154.00g/ L),vitamin D insufficiency(median 137.00g/L),vitamin D deficiency(median131.00g/L),P<0.04],serum vitamin D levels and hemoglobin levels in patients with Crohn’s disease were positively correlated(r=0.427,P<0.01);The results of binary logistics regression analysis showed that vitamin D was a risk factor of anemia in patients with Crohn’s disease,and the higher the vitamin D level,the less likely it was to be anemia in patients with Crohn’s disease(OR=0.803,P=0.01).ROC curved analysis showed that when serum 25(OH).D >19.16 mmol/L,patients with Crohn’s disease were considered to be without anemia,and the predicted sensitivity was 64.3% and specificity was 91.7%.Conclusion: Vitamin D levels in patients with Crohn’s disease show a clear positive correlation between hemoglobin,which is an independent predictor of anemia in patients with Crohn’s disease,and the higher the vitamin D level,the lower the likelihood of anemia.Increasing vitamin D levels in patients with Crohn’s disease can help improve anemia in people with Crohn’s disease.
