ObjectiveThe purpose of this study is to observe the clinical effect of modified Gexia Zhuyu Decoction combined with external application of Traditional Chinese Medicine in treating chronic pelvic pain of qi stagnation and blood stasis and to explore the effective ideas and methods of this disease.MethodsA total of 78 patients with chronic pelvic pain of qi stagnation and blood stasis who met the inclusion criteria of this study were collected and divided into observation group and control group by random number table method according to the order of visits,with 39 cases in each group.The control group was treated with modified Gexia Zhuyu decoction,and the observation group was treated with external application of Traditional Chinese Medicine on the basis of control group.After the treatment,the therapeutic effect and score of TCM syndrome,the therapeutic effect and score of local physical signs,the VAS score of pelvic pain degree,pelvic lump and hemorheology of the two groups were compared before and after the treatment.The summary and statistical analysis were carried out,so as to evaluate the clinical effect of the treatment on chronic pelvic pain of qi stagnation and blood stasis by modified Gexia Zhuyu decoction combined with external application of Traditional Chinese Medicine.Results1.A total of 78 subjects were enrolled in this study,including 1 case excluded from the observation group,1 case excluded and 1 case shed from the control group.Finally,75 subjects were included in the study.Before treatment,there were no statistical significant differences in the course of disease,age,number of pregnancies,number of deliveries,and history of pelvic operation between the two groups(P > 0.05).The baseline data were basically consistent and comparable.2.Comparison of comprehensive efficacy: The total effective rate the observation group was 86.8%,the total effective rate of the control group was 73.0%,the difference was statistically significant(P<0.05).The effect of the observation group was better than that of the control group.3.Comparison of VAS pain score: After treatment,VAS pain score of two groups decreased significantly.The difference was statistically significant before and after treatment in both groups(P<0.05).The difference between the two groups after treatment was also statistically significant(P<0.05).4.TCM syndrome score and efficacy comparison: After treatment,the total TCM syndrome score of the two groups was decreased.The difference was statistically significant before and after treatment in both groups(P<0.05),and it was also statistically significant between the two groups after treatment(P<0.05).The effect of observation group was better than that of the control group in improving lower abdomen and lumbosacral pain,menstruation(volume,color,property),daixia,(chest,flank and breast)swelling pain,emotional depression or irritability,and the difference was statistically significant(P<0.05).The effective rate of TCM syndrome was 94.75% in the observation group and 81.1% in the control group,the difference was statistically significant(P < 0.05).5.Comparison of local physical signs score and efficacy: After treatment,scores of local physical signs in both groups were significantly decreased,and the difference was statistically significant before and after treatment(P < 0.05).Compared between the two groups,the score of the observation group decreased more significantly after treatment,and the difference was statistically significant(P < 0.05).The effective rate was 81.6% in the observation group and 67.6% in the control group,and the difference between the two groups was statistically significant(P < 0.05).6.Pelvic lump : After treatment,pelvic lump in both groups was reduced.The intragroup comparison and inter-group comparison before and after treatment in the observation group showed statistical significance(P < 0.05),while there was no statistical significance in the control group before and after treatment(P > 0.05).The results showed that the observation group had better treatment effect on pelvic lump than the control group.7.Hemorheology indexes: After treatment,hemorheology indexes(whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity,fibrinogen,red blood cell aggregation index)of the two groups were significantly decreased,and the difference was statistically significant before and after treatment in both groups(P < 0.05).Compared between the two groups,the hemorheology indexes of the observation group decreased more significantly after treatment,with statistical difference(P < 0.05).Conclusion1.Oral modified Gexia Zhuyu decoction has a certain effect on the treatment of chronic pelvic pain of qi stagnation and blood stasis type.Combined with external application of Traditional Chinese Medicine can further improve the clinical effect.2.Modified Gexia Zhuyu Decoction combined with external application of Traditional Chinese Medicine can effectively relieve pelvic pain,improve TCM syndrome and local physical signs,promote the dissipation of pelvic lump and improve hemorheology.
